My career has not even started, I'm just an intern. But I still would like to narrate a long story.I was posted in Surgery department for my clinical rotations, it was my first posting, I was extremely enthusiastic and curious. I love my Surgery department teachers, they were simply put in words- dilligently amazing.I was meeting patients and interacting with them too much, I use to give them my personal number when they asked for it.It was 14th of March this year, I remember the date because it was one of my best friends birthday and I chose not to meet him and attend the case. In the evening hours arrived an old emasciated man with his son and daughter in law, with complaints of increase in size of his belly with pain and he had not passed stool since 2 days. I was the one who examined him initially, I was able to independently feel the signs such as guarding, rigidity, absence of bowel sounds etc… I was confident about my diagnosis of acute intestinal obstruction. I was extremely happy.Then comes the critical thinking area, why should a person have the intestinal obstruction, one of the professors raised a high suspicion of malignancy (cancer) of his colon/large intestine.We advised for investigations and asked patients attenders to be quick. They were quick and it was around 9pm he was scheduled for an emergency surgery where we had to open his abdomen to find the site and reason for obstruction. We found signs of malignant disease, his abdomen was filled with fluid that appeared likely to be because of cancer. There was a mass which was stony hard at the site of obstruction.The person underwent Right hemicolectomy with temporary end ileostomy. Where a part of his large intestine was taken out and because of impossibility of anastomosing at same point a part of his intestine was let out from skin for defecation purpose. We sent the sample for histopathology lab for confirmation before starting chemotherapy.This is how it looks- (representational image from wikipedia)This was the horror, ileostomy, I was enthusiastic to take care of him, I did whatever possible to keep the patient comfortable.But it is a really challenging job to take care of ileostomy site if a patient is not co-operative. All he wanted was correction of his ileostomy.But according to the treatment protocols, atleast 6 week gap is necessary for a successful anastomosis. That is, joining the two ends of the intestine that were cut. There are several reasons for that such as leak, multiple holes(fistula) etc.On day 1 after surgery, I went early to meet him, I did my rounds and reserved him for last as I wanted give him more time. I greeted him and congratulated him for withstanding the procedure. I did the dressing and the moment I asked him to show the site, he was in tears. He considered ileostomy pouch as bad.I consoled him and reminded him that it was temporary, he was visibly upset. When the rounds started(rounds is a part where a team of doctors arrive near patient and discuss about case, it happens minimum 2–3 times a day everyday). He again requested for removal of ileostomy as soon as possible to our unit chief.Then days passed and histopathology report had arrived and it was confirmed case of adenocarcinoma of colon with lymph node metastasis. That is cancer of large intestine with spread to lymphnodes… that didnot bother him, he was upset with ileostomy site.He said- Saab Mera jitna tha Mai jee Liya, aur Jo bhi bacha hai jeena ache se jeena hai, ye gandagi nikaldo Saab pehle jaisa kardo. (Sir, I have lived my life satisfactorily, whatever days are left I want to live like same, please make it as before and put this bad thing inside)… and he requested every day during every round and our explanation was the same and it gets annoying after some time but I tried to not lose my calm and I tried to console in the most empathic way possible. 7 days later he was devastated and crying when I came for his rounds.In his words- “Doctor Saab ye nikaal do, kuch to Karo ye nikaaldo, Mai marjatu Saab lekin ye wapas andar dalo, ye gandagi hai, mere Saath me rehne walo ko bohot takleef hai, badbu aati hai aur ye gandagi hai. Saab ye ganda hai nikaal do…”Translation- Doctor please remove this, do something remove this, I want to die but put this inside again, this is bad, this gives trouble to people around me, it smells bad. Remove this, it is bad…I counselled the entire family that day.Then days passed, later he was discharged. Advice was given regarding application of silver paint around the ileostomy site to prevent skin erosion. He use to call me and speak to me on phone for few minutes every day/alternate day about his health.2 weeks later he arrived, lost some weight, and was visibly depressed about the bag. Same thing he had to say… Saab ye nikaaldo, ye ganda hai. (This is bad, remove this)…We advised for protein powders and healthy nutrition and advised him to gain weight so as to be physically fit for next procedure and advised him to get the chemotherapy at a cancer hospital.He was scheduled for another surgery- anastomosis.The day arrived when his wish was about to be fulfilled, but fate had something else in his bucket, on start of procedure he developed a myocardial infarction i.e heart attack on table, procedure had just begun and surgeons were shocked to see yet another growth that was inevitably going to cause obstruction later, again his belly was filled with fluid due to cancer, but procedure had to be abandoned and he was resuscitsted. He survived the heart attack, thanks to our anesthesiologist team.And sadly, when he was in ICU, he was devastated that the procedure failed. I met him, I congratulated him for surviving a heart attack, I wished him best of luck for next time.Then after becoming stable, we advised for procedure at a higher facility and asked him to get operated there.Unfortunately, because of cancer cachexia(condition where patient doesn't feel hungry, doesn't eat, and doesn't feel good) he lost weight and was unfit for surgery. He could speak only 2 words at a time, he was that breathless, probably metastasis to lungs. He definitely needed palliative care at higher center.He use to call me to seek help regarding the multiple things. We referred him to onco-surgeons(cancer speacialist surgeons).It was over 3 months and he was unhappy with the outcome. He was very sad. He use to call me on phone and cry about how sad he feels about it. I tried my best to hold my tears and explained him how it works.He always was very respectful of the doctors, but he had only one complaint- please put the bad thing back inside!It's over 5 months, he has not yet been operated for anastomosis, chemo was started. I haven't received his call in last 2 weeks, I will call him tonight.He is definitely the most difficult patient I have dealt with till now.Update:- I spoke to his son, the person is still not operated for anastomosis (his main problem), and the major issue for doctors to deal with is now his cancer cachexia which has lead to mostly secondary pulmonary tuberculosis (suspected). I have asked them to consult the cancer institute which is associated with government hospital in my city. How do I prepare a portfolio for applying for a Master's Degree in Graphic design ?

At minimum, I would recommend having a web-based portfolio that showcases the best 10 graphic design pieces you done within the last 5 years, nothing older than that unless absolutely unavoidable. Try to have NO MORE than 10 pieces unless the school requires more than that. Create 3–5 BRAND NEW pieces of design work exclusively for the portfolio if you do not already have 10 quality pieces, created on your own or through your employment and/or independent clients that you feel good about showing. This has to be your BEST work and it needs to reflect a high caliber of visual excellence, as well as some thinking and planning behind the visual decisions made - the people evaluating your work want to see how well and how advanced you THINK about the nature of design, just as much as they want to see your technical skill and visual knowledge of layout, colors, typography, etc. What 10, and ONLY 10, pieces would you feel comfortable showing if you wanted to land your “dream job” in a design firm or ad agency? Use that as the starting point to choose these pieces of work for a master’s program application.Wix.com, Adobe Portfolio and Squarespace are some options that come to mind immediately for hosting the portfolio.Please be advised that each individual institution may have DIFFERENT requirements, and it's important to carefully understand and follow the specific directions for EACH program you wish to attend. It may be time and cost-prohibitive to apply to more than 3-5 master's programs as a result, so choose your school carefully before you decide to apply - I would even suggest spending time, money and travel/lodging expenses and taking a personal visit to your top one or two choices and personally meet with professors and students. (Some schools, such as Yale, actually have a formal open house that they strongly suggest interested students attend.) Do this BEFORE you even THINK about going through the application process or preparing your portfolio.NOTE: There is a website called SlideRoom which seems to be an industry standard nowadays for universities' applications. I entered the grad program in graphic design at NC State about 4 or so years after my BFA from Arizona. That means I had over three years of real-world working experience in the design/production field under my belt before undertaking graduate study, and that gave me a nice foundation and perspective. Most of my classmates also had extensive employment experience also, some coming back to school after several years in the workforce and having relatively successful careers.Also, to reiterate and expand upon a previous point: it’s a good idea to consider your options among at least three or four different programs. There are sometimes vast differences from program to program as far as philosophy and “personality”, so it’s critically important to shop around, ask tough questions of the faculty (because they WILL be asking tough questions of you right back) and please try and take an in-person visit to at least one of your top choices.Also: consider this: could you achieve your career goals just as easily WITHOUT the expense and life-altering uproot required of a master’s program? Perhaps you can take a portfolio development course at your local community college. Maybe there are education resources online (some of them offered at no charge) that you can take advantage of. Check into anything offered through professional organizations in the design/advertising professions in your area, like AIGA or local advertising clubs. Many of the big names that students study in the graduate programs have published writings easily available in a decent university library, or even online. If you can afford it, consider attending one of the creative/design/advertising/typography conferences (such as AIGA or HOW Design Live) held each year in different cities.Just something to think about as you consider graduate study.
