North Korean dictator Kim Jong Il died from a heart attack, in 2011, sparked by a "fit of rage" over a shoddy construction at a hydroelectric power plant, South Korea media claims.A report in the Chosun Ilbo, a conservative newspaper in South Korea, says the late "Supreme Commander" blew his top after being told about a massive leak at the hydro-electric dam in Huichon, Jagang Province.Jong-II considered the plant, located some 75 miles north of the power-sapped capital of Pyongyang, crucial to the reclusive country's energy future and had visited the site eight times since it was rushed into construction in 2009, the report said."After being briefed about the leak, Kim Jong Il lambasted officials and ordered them to repair it," an unnamed source told Chosun Ilbo."He rushed to make an on-site inspection of the facility unable to contain his anger and died suddenly."At the time of the leak, Kim Jong Il was already fuming over reports of defects at steel and textile manufacturing plants also considered "poster projects" of North Korea's emergence as a "powerful and prosperous nation," the newspaper said.http://www.nydailynews.com/newsThis was alleged from several sources but never confirmed by the North.​​​​​

I was in my fourth year and interning under a Judge at the Calcutta High Court.There was an interesting matter which came up before him one day. A petitioner had filed a petition attempting to attract the attention of the justice to the mismanagement of funds in a local high school run by the state education department in some rural district of West Bengal.Hon’ble Mr Justice called the representatives of the state department and had them explain the position before him. Being dissatisfied, he called for the files pertaining to disbursement of funds.The file came and along with it brought heaps of laughter. The papers disclosed that a hefty sum had been granted by the state for the creation of a second storey of the school building. A completion report was filed thereto signifying that the construction of the second floor was indeed complete. The papers were, however, not very coherent. The judge expressed displeasure and ordered for a site inspection.We expected something like this:What the site contained was something like this:(image credits: Google)The team returned and reported that the ‘two storey school building’ was a hut. It was a temporary shed housing two classes of school going children.I didn’t know whether to laugh or cry, The judge surely did. Two officials of the education department were immediately suspended and strict action was taken against those found guilty thereon.

Yes, they do but on a limited basis.The American Institute of Architects document B101 Architect and Owner contract agreement, describes among many other rights, duties and responsibilities of each of the parties, the amount of times that the architect should attend the construction site.Evaluations of the workThe role of the architect following this agreement is to supervise the work of the contractor at intervals appropriate to the stage of construction in order to become familiar with the progress and quality of the work completed.This follows the purpose of making a professional judgement of the work and determine that after it is fully completed it will be in accordance with the contract documents.However, the architect is not required to make exhaustive or continuous on-site inspections to check the quantity or quality of the work. That is the role of the general contractor who appoints a superintendent for the task.On the basis of the site visits, the Architect shall keep the Owner reasonably informed and report to the Owner known deviations from the Contract Documents.When the contractor requires money for the work completed, he submits an application of payment which must be approved by the Architect.The Architect must then inspect the site to determine the level of completeness of the work and certify or reject the application of payment.The architect cannot interfere with any of the means and methods employed by the contractor but is allowed to reject work for non conformance with the construction documents.It is also not the responsability of the architect to observe safety at the construction site.It goes without saying that the contractor needs to grant access to the Architect and Owner to the construction site. These need to retain proper insurance to cover them in case of injury during their professional work.Project CompletionThe architect needs to conduct inspections for the aformentioned reasons but also to determine the dates of Substantial Completion (when the building is ready to be used) the date of Final completion and issue a final certificate of payment based upon conformance of the work.In general, sound professional judgment should determine the timing and number of visits to the site in accordance with the type and nature of the project.If the parties choose to do so, they may designate the number of visits under written agreement.Problems may arise when site visits are limited. It is in the parties best interest to retain the architect to make sure he provides and adequate number of site visits.Source: AIA
