Way back when women were “knocked out” during natural childbirth, how was the baby delivered safely if the woman was not awake to push?

I was a childbirth educator in West Alabama during the late 1970’s. In one class, a nurse client, who worked in the pediatric dept of the state mental hospital, told me that many of their children were delivered by high forceps (a device which grasped the babies heads and pulled them out).Even then, the only OB operation in town, a group of five doctors known as “the fifty fingers”, gave narcotics to women in labor and then applied gas- right at transition, when Moms should get the urge to push. As Moms were too sleepy, their pushing efforts, in many cases, were ineffective and forceps were used.Narcotized Moms delivered narcotized babies which had no urge to nurse. They were kept in the nursery as a rule “until Mom is awake and wants the baby”. Nurses fed water and formula to their tiny charges, which inhibited Mom’s milk flow. Moms were then persuaded, in a time when few women nursed, that “some women just don't have a good supply of milk”. This was confirmed by doctors at the first well-baby exam, when current weight was noticed to be lower than the birth weight. Infant-maternal mortality at that time, in that area, was as high as those in the country of Chad.I was pregnant then, with my second child, and was seeing a midwife seventy miles away in Birmingham. Though I had a c-section with my first child, the hope was that I could yet have a vaginal delivery. Do you need any doula training, if you've operated as a labor and delivery RN for many years?

If you want to get certified, then yes, you will need to complete the training requirements of either DONA International or CAPPA . I believe that DONA waives the childbirth education classes if you have been a L&D nurse but I don't believe that CAPPA does. There are other organizations as well, such as International Childbirth Education Association | that provide certifications. Most of these organizations have online classes and requirements for training.With that being said, certification is not necessary to label and market yourself as a doula. I am not aware of any states that regulate doula services because doulas do NOT provide any medical advice. You may want to read up on different methods of labor management because that will be your skill set and not all L&D nurses are familiar with all the different methods.
