Take this document(s) along with you when you visit Vet for Health Certificate. Also, do check with destination country’s Animal Quarantine dept by sending an email whether they want whole vaccination record or just the recent one.Step 2:Health Certificate: You will find the form for the Health Certificate on the USDA website. This form need to be filled by the Veterinarian who examines your pet. This document is going to be the Health Certificate once filled by Vet and then stamped by USDA. Don’t forget to ask the Vet if he/she is USDA approved Vet. One more thing, note down the weight of your dog, you will need that when book airline ticket.MicrochipMy dog already had microchip implanted on his neck. So, I was bit relaxed. I am not sure if it is required or not, but you will find a column asking about microchip number on the Health Certificate.
