As a doctor, what's the most shocking thing to transpire while delivering a baby?

When I was in Anesthesia Training in the early 1970’s, it was in Appalachia. Because most patients presented without prenatal care we saw many sad and deformed births. I attended a C-Section performed on a woman who had been in labor for several days.She walked down out of a “Howlar” (hollow) for about a half a mile and then hitch-hiked a ride in a pick-truck to the Harlan Appalachian Hospital.When the baby was delivered by Section, the entire room grew quiet. All I heard was a cat-like screaming from the baby warmer. I went over to see if my attention was needed. I was immediately told not to do anything as there was really nothing that could be done.The baby was an Anencephalic. Born without a head. It is a neural-tube failure to close. This particular presentation looked as though someone had cut off the face and plastered it down between the shoulder blades. No neck, No Head.I took about an hour for the baby to finally die. I had nightmares for a while after that.—————————————————————————————————————-Another time at the same Hospital, a woman came in with no prenatal care who had also been in labor for several days.As they were washing her abdomen for the C-Section, I mentioned to the OB-Gyn that I could strangely see the outline of the baby more clearly than I had ever noticed before. It was like she had no subcutaneous fat.He took note of that and was careful with his incision. Immediately upon opening the skin layer the baby was delivered without opening the Uterus.No one in the room had ever seen an ectopic pregnancy go to term before. The entire pregnancy was outside of the uterus and the placenta was attached to the intestinal omentum.Not really knowing what to do the Surgeon clamped the cord, delivered a healthy baby. Stuffed the rest of the cord and placenta back in the abdomen and closed without attempting to deliver the placenta. He had no idea what to expect next.Both the mother and the baby survived and did well. The mother returned less than a year later and delivered a normal Uterine Pregnancy by C-Section, without incident.The Surgeon could find no evidence of the previous placenta or attachment. Her body had absorbed it and her anatomy look completely normal.She would have bled to death if he had attempted to remove the placenta at the first pregnancy. Just Dumb Luck.
