As a doctor, what's the strangest case you've ever seen?

This was a case that I encountered about fifteen years ago. It involved a two year old hospital patient, a little boy I had delivered and I knew him and his mother well. Mom brought him in one day after he had been running a high fever (103–104 F) for 3–4 days. The fever came on like clockwork, twice a day. He was otherwise behaving normally so she had assumed this was a virus that he would recover from (a very reasonable assumption in this age group for an experienced parent). The only thing she had noticed, in addition to some fussiness with the fevers, was a rash that would appear around his knees and ankles as well as his wrists, only when he was running his fevers. I had managed a couple of cases of Kawasaki’s disease in this age group (a rare though not unheard of condition) and began the standard treatment, which consists of intravenous immunoglobulin. With Kawasaki’s (we still don’t know for sure what causes this illness and there is no good lab test to diagnose it) the fever will rapidly resolve and stay normal, and this is a fairly reliable sign that the diagnosis of Kawasaki’s is correct. However, this time the fever did not go away. Furthermore, we observed the exact same thing that Mom had. We wound up consulting infectious disease, who was also puzzled. We wound up sending him down to Indiana University in Indianapolis for further evaluation. The final diagnosis? Systemic onset of juvenile rheumatoid arthritis. This is a very rare form of JRA and I have not seen it since. This little boy did well after starting the right medication and he eventually became the poster child for the Indiana chapter of the Juvenile Rheumatoid Arthritis Foundation (which has been shortened to the Juvenile Arthritis Foundation).
