She was awake and crying and her tears were actual blood streaming down her face.I was about to go off shift after working over night and the nurse told me they put a “sick one” in room 20. I looked at the clock. My relief was due to be there at any time and I was a little reluctant to start a complex case since I was about to leave. She had a cough yesterday. She didn’t eat dinner because of nausea. She did hardly sleep because of shortness of breath. She seemed confused this morning and so her friends brought her to the ER. She looked at me with blood streaming down her face and said “I don’t feel good. Help me … Please.”Her vital signs signs were bad. Her blood pressure was low and her pulse was high and her blood oxygen level wouldn’t even register. I had her moved to a trauma room immediately. As I walked beside her bed my relief doctor joined us. I filled him in. He said “do you think it could be something like E. bola?” Damn. I had been thinking the same exact thing but had sort of convinced myself I was crazy. I told all the nurses to get gowns and masks as we rolled into the trauma room.They moved her over to the bed and she stopped breathing. While my partner intubated her and placed her on a ventilator I started a central line. As I advanced the needle into her subclavian vein I aspirated pus. Shortly after this, my partner tells me she’s in full arrest.We started CPR. The nurses started to get IVs and give medications but she would just bleed from every IV stick site freely. We did everything we knew to do but she never responded. By the time we were done she was bleeding from her mouth and eyes and bladder and multiple IV sites. It was horrific.I called the medical examiner and insisted on an autopsy. The impression he had was that she died from pneumonia with sepsis and Disseminated Intravascular Coagulation syndrome or DIC. When I reflected on the diagnosis provided by the medical examiner my head agreed with him. Sort of. But, my heart kept coming back to that young girl crying blood who died right in front of me and I wasn’t so certain.
