My children's school wants me to sign a form allowing them access to their Medical Records. Is this too intrusive or am I overreacting?
It is not uncommon for schools to request you to sign a release form that a representative of the school is allowed to seek medical treatment for your child. Double check you are not confused as these forms are sometimes titled “Medical Release” (which could very easily sound like a medical records release.) These forms are particularly important if your child will be off campus for field trips, sports, tournaments, etc. or you work far from the school. If your student has a medical issue, you’d want a teacher/administrator to be able to get them treatment. You might also be authorizing any treatment facility (NOT the school or teachers) to contact your child’s primary care physician for necessary medical information to ensure that your child is properly treated. Again, this is important for safety reasons and I personally always sign off on these forms.If this is not the case, I’d inquire why the school would need access to ALL medical records. It’s not uncommon for a school to ask for limited medical information signed by a doctor for various purposes, but I’ve never heard of a school asking for a full medical records release form before.
What is the most bitter truth that you had to reveal to your patient as a medical professional?
This happened early on in my career and studies.I was doing a practicum in mental health, working at an on-campus mental health resource center. One of my patients was a very involved student.I did the intake and scheduled her from weekly hour-long appointments. Student said that she wasn’t worried about her drug use and only used coke occasionally. Classes were okay. Nothing too serious - simple adjustment stuff. The students received the services at no cost.First week, her appointment time comes and she is fifteen minutes late. She came in, saying that she had almost forgotten. And then, with fifteen minutes left in the hour, she said she had to go because she had something with her sorority.I gave her an assignment to develop a weekly schedule on paper before the next appointment. I also advised her that she would absolutely need to be on time for her next appointment.Next appointment, she arrives about five minutes late. Better but not on time. She tried to laugh this off saying that she really need to work on time management. She had not done the assignment so, despite her protest, we spent some time during her hour working on the schedule. As we were doing this, she kept saying that she would do it and I advised her that, if were to continue, she needed to consider what she wanted to get from our time together.Next appointment, no show until there was only fifteen minutes left in her hour. She got very upset when I would not reschedule her for an appointment that week. We spent about ten minutes exploring whether or not she wanted counseling. I asked her straight up if she had decided to check out the services because getting counseling seemed kind “cool” or if she really wanted to deal with some issues. She had no answer.Next appointment. Half hour late. I advised her that I was terminating her as a patient. We had a waiting list. She went ballistic. She advised me that I couldn’t do that. She could also give no reason as to what she wanted counseling services saying it was just something she thought she should do.Knowing that none of the on-campus resources had openings, I advised her of numerous off-campus resources. These were fee-for-service, sliding scale places.She seemed offended and said wouldn’t go to one of those “charity” places. I offered a referral to a private counselor and had her sign release forms for me to send records if she chose to see that person. She said she would think about it and let me know.I never heard from her. At the end of the term, I was able to sneak a peek at the grades of some of the patients that I had seen. Her grade point was well under a 2.0.I learned a great deal about dealing with manipulative patients. At the point that all of this happened, I had only completed about a half a year of my training.
If your parents own a spare apartment in a city (they don't live there technically), can you live there counted as living with parents freshman year of college, if your college says freshman must live on campus or with parents near by?
You don’t state anything to the college.Your Parents have to sign a Release form that you are living under their supervision.Now the college is absolved of a Lot of responsibility for you as a freshman, and you are off-campus a lot and not gaining the exposure to the other peer students that you should be gaining, and you are probably traveling to/from your apartment during the early hours of the morning or the late hours of the evening, when things may not be too pleasant in the city.,…….Think through all of that, and then let your parents rent out that apartment and you should live on campus…..
