Per this Texas shop’s page, it appears that minors (under 18) must have parental consent. The same information appears on multiple sites.“Minors ages 14-17 (some ear cartilage piercings) and 15-17 (facial and navel piercings) must have the following THREE ITEMS which will need to be copied upon arrival each and every time you get pierced (we do not keep these on file):​1. Minor's birth certificate OR notarized Parental Consent Form (downloadable form at the bottom of this page).This is NOT a form of ID, if the parent/guardian providing their ID is not named on the birth certificate court documents showing guardianship or the notarized Parental Consent Form will be needed.2. COPPA currently stipulates several "highest levels of parental verification" methods including:- credit card charge- "faxback" where a parent downloads a file, fills out the consent form, signs it and faxes it to the company (e-fax or hard copy)- driver's license, soc sec # verification- 800 number where a parent phones the company to verify parental consent'e-mail plus' is not considered the "highest level of parental verfication" but can be used under particular circumstances. 'e-mail plus' requires sending a parent a registration email with the child's registration information, including pin or password and any personally identifying information/data collected. There really is no single place to obtain a Parental Consent Travel Certificate. In cases where a minor is travelling alone, on a school trip where their parents are not present, with a non-guardian adult, or with a single parent where the parents are divorced, you can find a form in many places on the internet. The main thing is, the form gives your signed permission for the child to travel without you and many places, such as school or the airlines will want a copy with your signature on it, so they have your acknowledgement that the child is permitted to travel.If you google “Parental Consent Travel Certificate”, you will find many places that have forms created that you can just download, print, fill out and sign and send with your child for their travel.
