Applications (and offers of admission) are valid for one year from the term of original application; however, deferral requests must be made before the start of the term of original application.Application Fee. A nonrefundable $65 application fee. The application fee may be paid by check, money order or approved credit card. McNair Scholars must submit a letter from their McNair Program Director verifying their status as a McNair Scholar in good standing to receive the fee waiver. Waiver request documents should include the applicant’s full name, address, date of birth, application semester, the name and title of the verifying officer and date of the request. Applicants for permanent residency must submit a copy of their I-485 indicating that an application to adjust status to permanent resident is pending with U.S. Citizenship and Immigration Services.IF ADMITTED - Bacterial Meningitis Immunization. TEC 51.9192 requires all students under age 22 entering an institution of higher education to provide current proof of vaccination against bacterial meningitis (an immunization given within 5 years of the date of intended enrollment) or meet certain requirements for declining such a vaccination. The original TDSHS form required for students declining the vaccination must be submitted to the Office of Admissions. For graduate students who are in a distance education program, they will need to fill out the Petition to Waive Bacterial Meningitis for Distance Education and submit it. I know personally how extremely frustrating it is when a parent is woken up in the middle of the night to his/her toddler or young child standing next to the parent’s bed, tugging on the blanket, crying and whimpering, as the parent is being pulled out of the deep dark recesses of sleep, only now to awaken to their young child’s distress.You sit up and reach to turn on the nightstand light and assess the child’s condition. Okay, so the child is seen pale, flushed, and lethargic, hot or cold and clammy, and complaints of head and eyes hurting, but was able to walk to you for help. You feel for a fever and the child is burning up! So to confirm the temperature, a thermometer is used and reads 104.2 F or 40.4 C. That’s hot. Maybe recheck another once or twice.Meanwhile, most parents are considering taking their child in for medical treatment. It is extremely important to know how to obtain an accurate temperature and to accurately dose using over the counter antipyretics Just these two considerations can either allow you time to see if the fever will respond to the medication. Given orally it takes ~approx 1.5 hrs to take complete affect. Meanwhile, start hydrating the child. If the child does not respond to being medicated, esp presenting with signs getting worse, refusing oral hydration. It’s time to head to the ER.If the child begins vomiting, do not offer anything orally, until the child has not vomited for at least 15 minutes, then you may offer a tablespoon of pedialyte or flat soda.. preferably sprite..(shake the soda good to decarb). Do not offer water or juices. If the child holds that down, then do another round of the same. Until you are comfortable that will stay down.Stay collective as you think through what you need to take with you to the ER. Make a note of signs and symptoms the child presented, temp readings, medications given, last time of food/fluid consumption. Bring insurance and ID info. Have a list of past history, the child’s medications, list of allergies, if known, the child’s PCP, and if convenient to obtain on the way out the door..immunization record. Bring fluids with you. Don’t forget comfort blanket and his/her favorite teddy bear:) The child can have heat/cold intolerances and start shivering violently. Children can make turns south quickly, such as with sepsis, meningitis, bad nasty viruses, bacterial, fungal infections. With children..don’t mess around.Although, your child fits the criteria for the ER, and you made the right decision; be prepared for a long wait, esp, if the ER waiting area is full. Emergency Rooms take patients based on triaging, that is, seeing the worst or the more critically ill first. Keep assessing the child for worsening conditions and report them immediately, while in the ER waiting area, as this may bump the child up in line.Remember to go with an attitude that you may have a wait ahead of you…this helps to keep frustrations in check. Sometimes, the constant complaints and crying from the child can add to the dilemma of frustratingly having to wait. Offer as much comfort as you can to your child, while being grateful that your child is only waiting and not being seen fighting for life:( The crying child, I don’t worry so much about..it’s the child that isn’t crying, I worry about.

I have, over the years, searched through cemeteries for genealogical purposes.It struck me, as I went from newer sections to the older sections that the number of children buried comprised a much larger fraction of the headstones in the older sections.Some of the difference is improved water supply, some the development of sulfonamides and antibiotics; but if you look through records you find that immunizations have changed the proportion to a much greater extent.Old newspapers with obituaries are a good source for those interested.Many of the younger physicians have never seen varicella, rubella, measles, bacterial meningitis, polio, smallpox, diphtheria, tetanus etc. Those of us who are older have seen them (perhaps not smallpox). In the future, infectious hepatitis may become rare. Cervical cancer and HPV tonsil cancer may become something of only historical significance.Those who wish to deny their children immunizations should be given the materials and lesson plans to ensure a good home education.
