Many doctors are very uncomfortable with Prescription Drug Monitoring Programs . They are very invasive . Many states allow law enforcement agencies to go through them with out a warrent.PMDPs are not limited to opiates . Every prescription is sent to a PMDP . I know this is a fact . I am a doctor .When I prescribe in the ER I can see ALL of your prescriptions . It can be very unnerving . I alresdy know what meds you take . I get this from patients or a person who is aware of their medical history . No. I do not need a life story . Just cuurent medications , allergies or a dangerous reaction to a med . Interestingly , I find medications used instead of a opoiod to have very serious If not life threatening reactions . I prefer you tell me and work with me . I do not need The CDC , law enforcement agencies , anti opoiod organizations such as PROP and its fanatical mouth piece Andrew Kolodny telling me how to do my job . A former NYC jail system psychiatrist has no place in pain manegment . He well known for pushing a dangerous and very expensive drug called subutex ( Buprenorphine or its combination with naloxone Suboxone. Which I've had the frightening experience watching as anesthesiologists could not induce anesthesia . We had to sedate the patient with a IV Ativan to keep them from recalling the horror and at least keep them calm as humanly possible . One we could use Ketsmine it was not a nice ten.I digress. . I've seen a few doctors investigated by the DEA which has no place in medicine . It was created by Nixon to go after his political oppenents and minpeities . I am one . I despise being questioned when I prescribed anything I believe is needed . I have seen very few drug seekers . The ER is not a cost effective way to get drugs . The addict on the street is far less expensive .Doctors also are second guessed by insurance companies .anh cannot eve prescibe physical therapy with out being harrased . They lean fast if you prescibe duloxitme ( Cymbalta ) Lyrica , or Gabaemtin ( Neurontin) you will not be questioned . Yet all are far more expensive than morphine and physical therapy . Then any opoiod is going to cost less . They are all generic . I prefer to use morphine and diazepam . It's very safe when used in therapeutic doses . Untill about 2016 when the draconian unscientific and based on PROP lies CDC opioid guidlines . I was able to use my experience , education , and judgment. Then the CDC wrote PROP's lunatic fringe anti opoiod guidlime . Non MD regulators . insurance companies and law enforcement agencies turned them into defacto law .Now doctors have to justify to people who are not doctors , are no where near as educated about medication and have never seen a patient why we prescibe a opiod . I can prescibe all the cymbalta I want . I can prescibe all thf Lyrica . Both have a low ceiling and are very difficult to withdraw from . The anti opioid fanatics and drug warriors could card less . I've seen people come into a ER taking a near learhal dose of so gabaergics or SNRIs and NSAIDS . They are revived and need to be intubated to breath , they need to be catheterized to both urinate and defecate . . They beech 24/7 monitering . This is not from a intentional overdose . This is from using medications as prescibed !Lyrica , cymbalta and NSIADs are not pain relievers . They are used tp control seizures , clinical depression and acute inflammation . NDAIDSare not pain relievers . They will not relive pain due to CRPS , EDS , Spondylitis, nueralgia. Migraines , MS, Fibromyagia, thoracic outlet syndrome , and just about any cause of chronic painSNRIs ,SSRIs , Gabaergics , and anti psychotics are not pain relievers . They all have very dangerous side efexts . They can make life hell for pain patients . Yet we force these drugs on them .If doctors do not push epidural injections , SNRIs , gabaergics. Before prescbing even a low dose of opiods . We are questioned by insurance . We will be recored in a PDMP and possibly face criminal charges .Yet junkies are dying . Not from prescription medication . Not from the mytical dr feelgood . No they choose to seek out heroin laced with a Fentanyl analogue . Thinking they will get a better high . They won't . Fentanyl is not heroin . It is not even chemically related . Fentanyl is a synthetic opoiod agnonist . It is a very safe and effective medication when used as intended . I prefer to use it when dealing with trauma . It works .Pain manegment doctors have used it in a transdermal patch since the 1990s it now is very difficult to abuse . Yet thanks to anti opoiod zealots and the fabricated statistics from the CDC it's becoming hard to prescibe . The cruel irony is it is also hard to prescibed physical therapy . I've seen a year limit of four weeks . Thrn we wonder why pain patients are angry , suffering and despise addcoe. . Who get taxpayer funded injection sites taxpayer funded rehab . Laws mandating insurance cover rehab which has a 98 % failure rate . Most addicts return to abusing within three months !Yet a doctor who wants to relieve pain and suffering is told no you can't ! They have a .002% chance of addiction . Less than 2% of pain patients become addicts .Then we need to ask is it Thr governments place to come between a doctor and patient. Is is the governments place to demand doctors act like cops and drug test and pill count ?Is addiction a problem government had any business in ? Is less than .001% of the population so important we must force law abiding people and their doctors deal with Orwellian , Kafkaesqe laws ? Which give the government vast power to intrude on the most intimate parts of our lives ?I don't want some do gooders or authortarian police state telling me what I can prescibe .I dong want them telling anyone what they can and cannot put into their bodies .We havd tried this with the prohibition it was a failure . We cannot legislate what people will do with their bodies . We cannot stop people from getting high. In Colorado we are benefiting from legalization of cannabis . We need to make Modt drugs avialble . It not only makes my life less stressful. It reduces taxes , reduces crime, Will make Heath care much less expensive . If you don't need a prescription . You won't take up limited time in a EE or doctors. Office . Only those who are unable to find a way to treate what is causing them to feel pain , discomfort, or other medical problem will see a docror . Reducing th costs and time you spend .It's called harm reduction . It's admitting we will never stop all junkie ms and druggies who choose to get highThr reason doctors are afriad is simple they do not want to go to prison for relieving pain and suffering . That is a cruel and inhumane choice .
