What is the craziest programmer epic fail you know of?

The Explosion of the Ariane 5Background:On June 4, 1996 an unmanned Ariane 5 rocket launched by the European Space Agency exploded just forty seconds after its lift-off from Kourou, French Guiana. At an altitude of about 3700 m, the launcher veered off its flight path, broke up and exploded. The rocket was on its first voyage, after a decade of development costing $7 billion. The destroyed rocket and its cargo were valued at $500 million.Reason:A board of inquiry investigated the causes of the explosion and in two weeks issued a report. It turned out that the cause of the failure was a software error in the inertial reference system.Specifically, a 64 bit floating point number relating to the horizontal velocity of the rocket with respect to the platform was converted to a 16 bit signed integer. Now, the largest integer storeable in a 16 bit signed integer is 32,767 and the floating point number which was converted had a value greater than what could be represented by a 16-bit signed integer. This lead to software errorThus, sometimes wrong data type conversions can lead to loss as huge as billions of dollars !!!Edit 1 : I was adviced to insert source of the information, in the comments. So, you can find the detailed news here.Thank you !

Why do most developers say that Python should not be used to build mission-critical software systems?

I am a professional consultant in the safety of mission-critical systems (specifically aviation systems). Also, my postgraduate degree dissertation was Python based. The whole issue is whether one finds bugs at run-time or at compile/build time (if such even exists for the language in question), and the extent to which bugs can be discovered through testing.Python is absolutely magnificent for getting (relatively) simple programs up and running quickly, provided any bugs will be detected quickly. I love it, and so do a lot of other people using it for non-critical applications or for “glue” between critical applications. But for the systems I deal with, which can kill a few hundred people at a time if they go wrong, it’s not enough for the system to work, I have to be able to *show* that it will work. And I can’t do that through testing, except for the least mission-critical applications, because the state-space is just too vast to test; I have to do it analytically. And that’s damned difficult for Python (or for any of the C family of languages too, such as C++. C# and Java derivatives). That’s why we really prefer “bondage and discipline” languages such as Ada subsets, Eiffel, and strict use of functional programming. It’s much *much* easier in those languages to use static analyzers to determine what the program actually does (rather than what the programmer intended). That pleases the regulator, my professional indemnity insurer, my clients, and ultimately me (because I fly a lot). No programming language is ideal for all uses; Python is a wonderful language, but mission-critical software isn’t what it was designed for and is not where it delivers.
