Application Form #7 Competency-Based Program: Fill & Download for Free

None of the answers posted here gives complete information.If you are staying in a different city for work, and renewing your passport there, AND you have stayed in 2 different residences in the past one year, you should mention both the address.The passport application form asks for 2 references (if you are staying in the present address for less than a year) and another 2 references from your village or town or city.I believe (as mentioned in the RPO site) i have to mention 2 references from my present address locality(same police station jurisdiction) and for the 'Reference from village or town or city' - I can refer friends and/or neighbours from hometown. Is this RIGHT? somebody clarify.

I had to fill my name as Siddarth “Gxxxxx”(caste name) which i did not want, now I have two identities , one in pan card(Siddarth Gxxxxx)and one in all the other cards like voter Id, aadhar etc which is Siddarth G.I also paid some 150 rs and applied for a change in name and asked them to change the caste name to my father’s name (both starts from G) but they said you do not have any other documents with your father name as surname so this change cannot be done, then how did they initially accept my caste name as surname while there was no other document with the caste name as surname? Clearly there are no proper rules.They told me to get signature from a Gazeted Officer, I have no Idea where to find one, so the money i paid for the change of name application is gone.Update: Today I saw my brother's pan card which reads Samarth G as his name. He went to the same guys who did not allow me to fill initials and my brother had no idea that initials are not allowed so those guys didn't say anything when he filled out the initials. I seriously don't get this.
