I was taking case history for a patient recently who was about to have an audiological examination and he reported that he suffers from severe tinnitus (perception of sound when there is no external sound source).He reported he that had suffered from this for many years and it severely affected his quality of life. He was a terrible sleeper, couldn’t concentrate on conversation or when reading books, had suicidal thoughts.I asked a few more questions about the tinnitus such as what is the sound that your hear? it’s usually a ringing or buzzing sound, but the patient reported that he heard chickens.Chickens? You mean ciccadas? I asked..No, chickens… as in buck buck buck chickens… all day long in my head.As i probed a little deeper the patient opened about about their history of child abuse. I wont go into the details but this person was was neglected as a child and made to live in a backyard chicken coop from the age of seven and was regularly abused there.The patient said they had a long history of seeking help to overcome their trauma but the sounds of those chickens had followed him into his adult life always reminding him of his past.

Recently, I have been having a stomach ache, dizziness and lightheadedness and a loss of appetite. What could all these symptoms mean?

As a Doctor of Audiology, I can tell you that all of the tymptoms you describe could be related to a problem with your inner ear. “Dizziness” is a catch-all term like a “bad back” that has multiple causes. But if your “dizziness” is a room spinning sensation (i.e. vertigo), that plus your other symptoms can reflect an issue with the balance portion of your inner ear. You need to see a Doctor of Audiology for a careful case history, balance assessment and Electronystagmography. This condition is not visible upon otoscopic (ear canal) examination and thus it cannot be verified by just a case history and a look into the ear.

What points should one consider in the first year while doing BASLP in India?

Shadow your seniors with the intent to learn from them. I regret having wasted that time.2. Cultivate the habit to read journals on topics you learn.3. Learn the local language of you are from a different state. It helps in developing good case history taking skills.4. Take up assignments from your seniors and staff.5.
