Emergency Consent Form: Fill & Download for Free

I am generally very critical about people who reject vaccines. However, this seems to be a cautious and perhaps justified reaction by TN health workers.Now I come across loads of clinical trial documentation. Someone sent me the consent form for this vaccine. If this form is real, here is my take:Generally successful completion Phase 3 trial is required for general public use. That seems to be missing here. What it means is that there is no marketing authorization.The above point is confirmed by the wording “in clinical trial mode”. So basically this drug is still under clinical trial.There is a concept called “Compassionate Use Program (CUP)” in which a drug may skip a level ONLY for compassionate use after weighing in risk/benefit. This particular consent form also states “under restricted use in emergency situation” which sounds more like compassionate use program.Drugs under CUP are NOT administered to general public. I am bit surprised why it is administered to general public.“Clinical efficacy is yet to be established”. I am also surprised why the word “safety” is not mentioned. Efficacy and safety go hand in hand. Why would a drug with unknown efficacy and safety be released for public?If someone asks me to take this vaccine, personally I would say no and keep taking my usual safety precautions till I see more concrete data. But then again I go through lot of clinical documentation. Not many people will have that luxury.What worries me is the anti-vaxxers will use such hurried introduction of a vaccine and build a case against vaccines in general.In short - as a person with overall good health, I personally would not take a vaccine that has been approved for emergency use. A young lad in his 20’s came to the Accident & Emergency Department with a hand-written note saying that he has been starving since 48 hours as he was unable to open his mouth due to a tightening of the jaws ! After examining the jaw, I felt that his jaw muscles were in spasm forcing the mouth shut ! And I suddenly uttered, “ You cannot be serious man, You cannot be serious !” , reminiscent of the expletives of John McInroe and I then decided to first relieve him of the spasm with a “ Maxillary-Mandibular nerve block” with a long-acting local anaesthetic, Bupivacaine ! So I explained to him what I proposed to do and I obtained his consent on the Consent Form and I proceeded to inject the local anaesthetic at the upper end of his jaws and I decided to wait for a few minutes for the effects !After about 3 minutes, he spontaneously opened his jaws and I again blurted out, “ You cannot be serious man, You cannot be serious ! “ Upon which he quickly replied, “ I am serious and I now can open my mouth and I am famished !” So I admitted him to the A&E ward for overnight observation and an immediate lunch and after 48 hours he now could even smile at me and shake my hands profusely thanking me !

For any operative type procedure, including the procedures I perform in the Cardiac Cath lab consent for blood transfusion is specifically named and covered by the procedure consent obtained prior to the procedure. For a Jehovah Witness patient who has chosen to refuse even the possibility or consent for a transfusion, that line about blood transfusion has to be scratched out, blotted out from the consent form and initialed by the patient and the operator signing the consent.So all blood transfusions for anyone in the hospital requires consent. In the Emergency Room where there is no relative available to sign a consent for an incapacitated patient needing transfusion, there is an emergency procedure where two physicians sign an emergency consent for a life saving requirement to cover for a moribund patient in an emergency setting.Thus for a Jehovah Witness - as for any patient - consent for transfusion is required. If a Witness does not give consent and a severe complication occurs that could lead to death then another attempt would be made to seek consent, but if the patient refuses then the hospital staff is in a very sticky place where they can be held liable potentially for the complication that led to severe bleeding and caused the death of the patient.I was once consulted by the Defendant’s lawyer, as an outside physician for expert testimony, regarding this exact, very complicated circumstance. To tell the whole story would be breaking my confidentiality agreement, so I can’t. But I certainly learned more from that experience how careful a physician and medical staff must be where issues of personal belief can impact on emergency treatment or any treatment.
