In the Philippines, board examinations for issuance of licenses to practice are provided for a number of professional work. Wow I don’t know where to really start. Hold on and stay with me. Guess I’ll start with.. I had contracted the Hepatitis C virus years ago. I didn’t know it then and contracted the virus before anyone ever knew about it or how to test for it. That didn’t come about until 1992. When I was young, I did a stupid thing and ended up in a Army hospital..my dad was a medic in the Army. There I received multiple pints of blood. My inner knowing tells me that this is where I contracted this virus. So years later, upon visiting a doctor unrelated to any of this…some lab work was ran and it come back that I had a Hepatitis B core antibody. Which was told to me..that, sometime in my life, I had contracted Hep B and my body fought it off and I developed antibodies against this virus and I was told at that time that I couldn’t contract Hep B again. Years later I was having some routine blood work done and to my horror, the discovery was made and it was official…I had Hep C, not a Hep B core antibody! Wow I’m racking my brain now..I have two little girls, what about them and my husband? I want to put you at ease, first people freak out when they hear Hepatitis C, scary it is yes, but, it is harder to contract the virus than say Hep A or Hep B. And we now have discovered Hep D and there is a Hep G, and who knows by now, what all alphabets are included. Within these different alphabets are different genotypes. I was fortunate to have a treatable genotype. Hepatitis C is very common and many many people may have it and don’t even know. The gold standard test used is a HCV RNA quantitative PCR panel and this test is costly. When someone contacts Hep C, the symptoms may be mild and you may just think you’ve had a bad cold or the flu, then it goes dormant and does it’s damage quietly and unbeknownst to you. Twenty years down the road and way sooner if drinking alcohol, a person can find their self in liver failure. I was referred out to a specialist..a gastroenterologist and one of my heroes! I underwent a liver biopsy to find out the damage done to my liver, because, I didn’t know how long I’ve had this and where I actually picked it up. I was fortunate that I was a Stage I…which is minimal scarring! Perfect enough for me and it’s was a treatable genotype!..even better, however, the treatment is horrific. Hep C and treatment, as well as, understanding these genotypes, is a very complex thing, so I will not be going there to make a long story shorter. Not only did I have to do treatment, I had to do treatment twice:( The first treatment was 6 months long only to discover that I had a 200,000 viral count still remaining at the end of treatment, which is an extremely low count, but even if it would have been one, it’s detectable and therefore can replicate and here we go again. The treatment was so hard on me that I let three years go by before thinking about doing it again. I then had another liver biopsy…hadn’t changed, still Stage 1. So now I undergo another round, but this time it was for 15 months. Through both of my treatments I worked full time and a clinic prn job as well. Amazingly .. I don’t know how I did it. I never had an appetite and nothing appealed to me. Much like chemotherapy. Food didn’t taste right and what sustained me was Kozy Shack tapioca pudding mostly. I did finally arrive and was cleared of the virus. That was years ago and have been tested many times since and still no virus on board;) Crazy shit happens to me…do I ever have normal colds or normal anything…No..there I’m picture perfect health. But crazy shit it is. So after all the hep shit, I finally address my allergies..it was starting to get pretty bad, my nose was running often and clear thin water dripped mainly from my right nostril. Actually, this has been going on as far back as I can remember, sometimes it dripped often and sometimes nothing. When I first addressed this, was when I was in first level nursing, of course when learning about all the disease processes, students have a tendency to become hypochondriacs. I think this was a premonition of things yet to come. At that time the runny nose was very persistent. Just leaning forward with my head down to read, I dripped a puddle on my page. But only the amount that was collected in my sinus cavity. I’d drop the same amount until it collected again. Nothing I learned about any disease process bothered me but one. And that was the removal of the pituitary master gland! That one freaked me out….for a surgeon to go into your brain through your nose..yeah! I didn’t want to sound stupid and I didn’t want my nursing instructors to think I’m now a hypochondriac and I sure didn’t want to get kicked out of the nursing program. But I did go to an instructor and asked her if she thought that it might be CSF fluid (cerebral spinal fluid) coming out my nose. She looked at me like I was crazy and I sealed my mouth for the rest of my tour through school. Ten years later, my youngest daughter, who had been under the care of an ENT for ear problems that was beyond a PCP’s scope to handle, she and I were in the kitchen and she had stooped down to put the dog’s food bowl down at about the same time I bent over to put the dog’s water bowl down. I never knew when I was going to drip…there were never any signs like feeling it along the wall of my nare. It just dropped out of the middle of my head when leaning forward rapidly into a little puddle. I had gotten so used to it, that I just stayed in place for those very few seconds until it stopped. Creating a little puddle on the doggy food bowl mat, my daughter wide-eyed look up at me and said “MOM Omg” I said…I’m sorry. It was easier to clean it up, instead of a trail trying to get to a sink or Kleenex or catching it in my palm. A short time later, out of the mouth of babes comes wisdom, my daughter had gone to google this..walks back into the kitchen and begins to ask me it the dripping is for one side or both. My dripping was from the right nare and she immediately said to me “do you know it could be a CSF leak” :/ A topic I try to engage many years back. She had an appointment coming up with her ENT and I thought it best to schedule myself at the same time. I spoke with this doctor and told him everything and he stood up and said to me “lean forward”…well, I don’t drip on demand. So I was sent home with a vacutainer and instructions to collect at least one ml. I had to keep it in the fridge and to collect an uncontaminated specimen was no small feat, by the time I made it to the fridge, I was empty. It took 4 days to collect 1 ml! Lol!! Finally I got it, took it in for testing and yes it came back thought to be CSF. My ENT called and said he was scheduling me to be seen by a neurosurgeon Three days later, I sitting in a neurosurgeon’s office. I knew this man from the hospital. He was a pedi, as well as, an adult neurosurgeon. Deja vu! He comes before me and asks me to lean forward, again I told him “I don’t drip on demand!” He was sort of nonchalant about this as he was sitting there before me telling me “yeah I see a couple of you guys a year and it can be like finding a needle in the haystack!”Okay:/ So he sends me for a procedure that is done in radiology that involves placing me into a trendelenburg position while shooting a cold contrast into my spine. Within one day the neurosurgeon himself calls me and loudly tells me that the leak was found in the Sella Turcica, which is that little Turkish saddle that houses the pituitary gland:. The skull was porous and he believes it was congenital or that I was born with it. The times I didn’t leak, was when my allergies created congestion or some swelling. The body makes approximately 500 ml of CSF a day and gets reabsorbed in the system. Everyday of my life, I woke up with a wet lungs and a wet cough. Oh shit. He proceeds loudly that I need to be scheduled for surgery and did I know that I was at great risk for meningitis?? Well yeah, I did consider that:/ Three days later I’m in the OR having my worst premonition come to pass! However, my pituitary gland was not removed, but it involved the same procedure to get there. The ENT and neurosurgeon together worked the procedure. The ENT had to open, the neurosurgeon used bio glue to repair and the ENT had to close. My nostrils were packed with huge tampons to the center of my head. And it was every bit as bad as I thought it would be and more.With all that stuff above being said, finally leads me back to your question. While at work one day, I was coming around a corner and a house keeper was mopping the hallway, I clearly saw she was mopping, so carefully, I skirted around and slowly preceded, as I stepped onto the wet surface, it was like I was stepping on oil, next thing I know I’m flailing trying to regain balance and.. way to late, my heads hits a non-supportive wall and it made a very loud sound…worse than it hurt. On the other side of the wall, is the ER waiting area. This little house keeper, who has walking disabilities is behind me trying to help me up, I’m sitting there trying to figure out how bad I’m hurt and where! And a dad, who heard the thud comes around the corner hollering “nurse down…we need help”. Omg if things can’t get any worse..and I’m scheduled for brain surgery in two days. I was so embarrassed, I got the hell out of there, security caught up with me as I was about to get on the elevator…I told him I’m okay and needed to get back to my unit. Then after the embarrassment wore off the pain set in and I found myself going to the ER downstairs and filing a incident report. I was taken to one of the back rooms while I waited for one of our physicians to see me. This woman physician, who I had the pleasure to come to know here in this room under these circumstances. After assessing me and me telling her about surgery coming up and somewhere along the way while giving my history, Hep C was brought up and I told her my story and that I received treatment successfully, she confided in me that she had contracted Hep B and it was not a treatable genotype. She had attempted treatment once and it wasn’t gonna happen and that it was very expensive and it is! She was from the Philippines and said to me as she put both hands to her face in disbelief. “Omg we used to handle and work with and in those cadavers with no gloves on when I was in med school”. She began to cry and I sat there numb as I was listening to this woman. I cried with her. She has a little girl, she was 4 years old at that time. This doctor, this wonderful woman said that she sees everyday as a gift from God. We ended while crying together, hugging each other and took each other’s hands and prayed out loud. I left there to go back to my unit to finish my shift transformed. I can’t even begin to put it into words.
