As an MD, how would you improve EMRs?

There are so many ways. Too numerous to count.Just to describe the problems that occur when I attempt to prescribe one drug needs a thesis.1-There are no blank orders. I cannot write in a drug if it does not appear on the chart. This generates untold episodes of hell. Allow me to give just one example: I try to prescribe mupiricin ointment. There is only mupiricin cream in epic. I write for the cream and put a note in the “note to pharmacy” (please dispense as ointment). Pharmacy contacts me that insurance will not cover cream and they cannot dispense ointment because the computer says cream. So they fax me a form to fill out. I fill it out. My MA faxes it back. I cannot get “ointment” added to the EMR without an act of God. A work order to epic is not enough. Every time I write this prescription I brace myself to experience this fresh hell. And this is JUST ONE of the MULTIPLE problems I face DAILY from just ONE aspect of using my EMR. Just one of many prescribing issues. How about a blank prescription? Too much to ask? Apparently so.2-DOCTOR WARNINGS are never useful or appropriate. Like anything with blue dye in it is flagged as an allergy because the patient vomited up codeine. Just…stop. How about (imagine the possibilities) embedding an interaction checker instead of doctor warnings that are constant, irrelevant, and mind numbing, requiring many angry clicks. AND YET…when there is a legit med interaction, like amiodarone or warfarin, there is…radio silence from epic. And the embedded UpToDate does not help so I need to whip out my iPad to do any real drug interaction research. But inside our brick walls with our shitty wifi, that is always dicey. The technology is there, God forbid we use it for something useful. Instead, I have to click click click the WARNING that the drug I prescribed is only for 7 days and not 365. Wow that is concerning….not. DANGER blue dye. OMG. Why?This is only two of at least 100 significant problems with JUST prescribing. It would take days to describe every problem.

What is something that people commonly did in the 1980s that would seem odd today?

-Hung out at the roller rink. (Do you guys even know what one of those is these days?)-Willfully depleted the ozone layer by using half a can of Aqua Net super-hold hairspray to keep your rad 80’s ‘do looking good.-Wore two or more Swatch watches on the same wrist. All in different bitchin’ colors, of course. (I miss my Swatches. I had two.) Also, wore leg warmers as a fashion item.-Made mix tapes, either by recording songs off the radio (often missing the beginning of a song if you couldn’t hit “record” fast enough) or from other tapes by using a boombox or stereo with a dual tapedeck. Sometimes from vinyl records, too, if you had a stereo with cassette and turntable. That was our version of downloading cool songs.-Went to the actual record store to buy actual records, because that was the only way to get new music.-Paid for groceries and other purchases with cash or a personal check. (Literally the only thing I use checks for these days is rent.)-Actually went inside the bank to get money, because ATMs were pretty new and most people didn’t have ATM cards. (Now when you see people inside the bank at the teller windows, you wonder what they’re doing in there.)-Called and spoke to an actual person when you wanted to order something from a catalog like L.L. Bean. Or else filled out the order form that came inside the catalog with the item numbers, sizes, colors, etc. that you wanted and sent them a check, money order, or your credit card number. Then you waited a week or more for the stuff to come.-Recorded movies and TV shows off the TV with a VCR. I always kept a stack of blank VHS tapes on hand in case I wanted to record something.-Stayed off the phone, or tried to hurry someone else off the phone, when you were expecting a call, because there was no voicemail or call waiting and no one had cell phones.-Answered the phone even when you had no idea who was calling, because… well, you never knew who was calling. We had no caller ID.-Roamed the neighborhood as kids with no adult supervision and no way of getting in touch with mom or dad, and no way for them to know what we were up to. CPS would show up at your house nowadays (especially if they knew some of the ridiculously dangerous shenanigans we got up to).-Bullies bullied kids to their faces, they didn’t hide behind a keyboard or try to ruin their entire lives. I’d actually take my 80’s bullying over the relentless and cowardly shit kids do now. At least once I got home from school they couldn’t get me.

This is hitherto the most important answer that all NEET aspirants must look at.Reading NCERT is an art which requires mastery. These two thin books are a compendium of everything you’ll need to learn for the NEET exam.I’ll share some methods which I had incorporated in reading NCERT during the time of my preparation. I’ve enclosed the images of only class 11th NCERT so that it’s suitable for every aspirants(11th standard/12th/12th passed)1-Read with the Paper Setter’s eye!Read each and every line of NCERT as if you are the one who has to frame the questions for NEET exam. Ask yourself after every line- what question can I frame out of it? I personally found 20–25 questions in the NEET 2019 exam which matched with the questions I myself had framed during my preparation.Eg-In the above example taken from chapter 1 of 11th NCERT the line marked with green can be asked in the form of fill in the blanks-Order and other higher taxonomic categories are identified based on the ____________________?2-Use different coloured highlighters and remember their colour coding.Yes it works too, guys. Not only it makes the text attractive to read but it also allows the brain to segregate facts of NCERT on the basis of level of remembrance.The colour coding of my highlighters werea). Green- For questions I had framed from high yielding and untouched portions of the NCERT. The questions which I was referring in point 1.b). Pink- For those lines which can be framed as assertion reasons( sad that AIIMS UG has been done away with😞)c).Yellow- For important points which are very common.d).Blue- For the name of scientists and their findings.e).Orange- For examples.3- Never Skip the Diagrams!!Every year around 10 questions in NEET( sometimes even more) are image based. For that you need to memorise the diagrams really well And even their labelling. If the paper setter decides to frame a tricky question he can integrate multiple diagrams in a single question!4-Do not skip the summary given after every chapter!!A lot of times, summary contains certain facts and points which are otherwise not mentioned in the main text. The question setter can utilise such points to trap the aspirants in a tricky question.For eg. Taken from Movement and locomotion chapter class 11th NCERT. “Muscle fibre” is the “anatomical unit” of muscle; isn’t mentioned anywhere else but in the summary. I remember this being asked in a mock test too!5-Do not skip the Introduction to the chapters just like the summary!Just like the summary, the introduction also sometimes have some valuable information which isn’t mentioned in the main text.Eg- Ernst Mayer being the Darwin of the 20th Century and the recipient of Triple Crown of Biology isn’t mentioned anywhere else. And I guess it was asked in a State PMT exam!6-Mnemonics for biology. Yes you heard it right.It is underrated but effective. It really makes chapters like Mineral Nutrition and Biology in human welfare very easy given the fact that a lot can be memorised using some mnemonics. You can even go non veg, if it’s easy for your brain to store ‘such’ content longer than others. Lol 😆Eg- I’ve attached my lame attempt 🙈 to create a mnemonic for a trend of elements in the chapter mineral nutrition.7-Everything in a single book. Helps in rapid revision!If you’re using a module or another supplementary book while reading the biology NCERT, note down the important points(high yielding and frequently asked) from such materials in the NCERT itself. A novel strategy! Believe me, you wouldn’t like spreading 10 books for revision on your table during the last months of your preparation. Utilise the spaces at the beginning and end of the chapter or the spaces at the corner of every paragraph.Eg- Some points which I had jotted down in chapter ‘Animal Kingdom’ and ‘Excretory System’...Some other points worth mentioning,8- Double digit NCERT read. It’s necessary if you want to score well in NEET biology. First, second and third read must be elaborative, line by line, point by point. Subsequent reads must be faster. From 10th read onwards you’ll be just skimming through the text and reading your highlighted points!9-Spaced revision. Once or twice a week. Every chapter must be revised after 1 or 2 week so that you don’t forget them.10-Mock tests for analysis of how well you’re reading.
