PDF Editor FAQ
When doctors say a good part of their day goes in documentation and paperwork, what kind of documentation are they referring to?
I usually end up writing down around 30 pages daily which includes clinical data, prescriptions, referrals to specialists, lab tests and things like that, I drain 1 pen per week, yes I'm in a developing country with no electronic medical records so everything must be in your own handwriting.
What is the hard part in the process of doing a patient referral for primary care physicians?
In my doctor's office, the problem seems to be making their electronic medical records system "talk" to the specialist's medical records system.
As a doctor, have you ever cried over a patient?
Am a nurse. I saw a doctor crying about a patient.I was working at this clinic through a nursing registry. It was a three-month assignment. This particular doctor was just a door down from the doctor I was assigned to. We knew each other well because it was not a large clinic. It was a fun place to work. She saw this patient who came in for the first time. He hadn’t been seen for years. He finally got Medicare when he turned 65 and his wife made him come in. She was with him.I didn’t know anything about that initial visit. But one day when I went to the lab to pick up the printed results (yes, it was that long ago, no electronic medical record yet - just paper charting). I looked through the stack quickly, and founded one. It was PSA that was very elevated. Having elevated PSA does not always mean the (male) patient has prostate cancer, but it’s not a good sign, and more work up needed to be done. I handed it to her and she whispers, “Oh no..” I said, “It’s not good, is it?” She said it’s the man she saw two days ago and during the rectal exam, she thought the prostate was quite enlarged. She was hoping it was not going to be like this, but she said she kinda brought it up to the patient and his wife. I excused myself and left the room. This was early in the morning and the first patients had not come in yet - I overheard her calling him on the phone and she told him it was very likely prostate cancer and she was going to send him to a urologist right away. She later came out to my desk (I was the early nurse) and politely asked me to process the referral paper.I looked at her and her eyes were full of tears. I was so moved by it, and I too, got teary. We got him seen the next day and they schedule a quick biopsy two days later. It was not as aggressive as she feared (he had surgery, and it had not spread anywhere else.)A month later he came in for a follow up, and brought us (just like the first visit), a box of donuts. I had a chance to tell him that I was so happy the problem was taken care of (he asked the doctor who was the nurse who got him that quick appointment and she introduced me to him, which I thought was sweet.)I am glad it had a happy ending.
