Social worker at the hospital, VNA, any home care agency, Elder Services, or the family doctor referral.

I would ask her doctor to send a referral to your local VNA for a home safety eval. They can assess your home and your mom to make some excellent recommendations. There are home safety checklists and the basics like removing all throw rugs, but a therapist is your best bet. They will be able to look at the big picture and come up with a plan that works for all of you as well as what you already and may need.

I have been practicing for 30 years. I have worked in hospitals, nursing homes, VNA, home care, schools (charter), out-patient clinics for adults, and now pediatric early intervention. My comments on hospital/nursing home are from time spent there 20 years ago. Things may have changed, so apologies in advance.Hospital: check on referral list, read charts, see new patients, conduct evaluations, swallow studies as ordered, fill out paperwork for Medicare/Medicaid etc. check on current patients, plan visit times around other therapies, tests, visiting hours, meal times. Chart notes immediately after sessions. Meet with doctors if needed. Attend medical staff meetings, rehab department meetings, manage department budget. On a yearly basis plan budget. Out-patient caseload seen in between. Hours range from 7:30 am - 5:30 pm (if doing meals for swallowing disorders). Weekends may be necessary for swallowing disorders. I was the only SLP on staff, so…Nursing homes: check with nursing staff re: new referrals and any new issues that have occurred with current caseload. Problem areas. Talk to nutrition services about feeding and swallowing issues, talk about dietary needs and texture/consistency, communication skills and problem solve. Conduct informal feeding evaluations, make referrals for swallow studies at hospital if suggested, talk to doctors when they come to visit re: challenging patients. Confer with other rehab staff regarding shared patients. Clinical notes are on special Medicare/Medicaid forms plus nursing home. Weekend work may be necessary as well as group treatment for certain disorders (meal time, cognitive therapies? Memory skills groups?)Out-Patient: Check schedule for cancellations, prepare materials for session, usually 30–45 minutes long, write session notes (SOAP formats), submit billing information to office manager, plan for group sessions and co-treatment with other disciplines if part of the program.School: Gather charts for the day (anywhere from 20–30 children), prepare materials, for each block of time, depending on age of child: go get child, or wait for student (6th-10th grade) to come to your area (I have worked in board rooms and basements and have always been totally amazed at how the work of SLPs is treated). Write notes at end of day, gather data, transfer to digital program 1X/quarter, write quarterly notes that coincided with grades. Take in referrals, start paperwork process to get: permission to assess from parents, assess, write report, make recommendations, send to parent, set up meeting to discuss results after determining when everyone can come (principal, or state representative, teacher, SLP, special Ed teacher/assist and parent). Before meeting, write the draft of the IEP plan, filling in ALL of the 30–40+ areas regarding education: grade, skills addressed, Core Leaning skills that speech therapy will apply to, # minutes loss to current educational time, # times per month, total # minutes per year, how you will measure progress, main goal, ST goals, where services will be held, will this child need special assistance in the classroom, will they till be able to get their elective classes, etc, etc, etc. Send request to parent, wait for their response, confirm meeting, hold meeting, discuss concerns, needs, plan, get approval of plan, finalize the plan, get signature, send to parent, begin services. Repeat this every three years. Schedule yearly meetings with parents to review plan, progress and certify that you plan to continue, add or discharge goals that have been met. Address new concerns of teacher/parent. Close IEPs that have been met. Note: there is a very narrow window of time to do meetings during the day because the teachers do not have much break time from theirr classes and if are called away, they need to have someone be there. Sometimes you are the perrson who has to help facilitate that, may be the assistant principal. Sometimes meeting need to be held before or after school. Most teachers do not want to stay afte because they have homework to grade and plans to make for their next day of work as well.Home based services: adults: depending on the agency, make sure that you have the forms for the pay or (Meicaid, Medicare - now I am sure they are online. But given the inconsistency of Internet and technology, forms). Gather materials: workbooks, cards, writing materials. Drive to homes: record start/end mileage for each home. Paperwork, billing forms completed at the end of each session. When I did this a caregiver had to sign that I was there. I know, things have changed. Sessions run from 45–60 minutes. You can do 6–7 per day depending on location and amount of drive time in between.Home based services children: I keep a stock of interesting toys in my car to stimulate interests in asking for something, so I double check on what is in the care and if I need to switch anything out. Gather charts together, anywhere from 5–7 per day. Drive, talk to parent about concerns, changes, prepare paperwork, work with kids, clean materials, put things away, off to the next house. Once per month quarterly reports on about 1/3 of my clients. Deal with e-mails, referrals, requests on a daily basis from a mobile office.
