Medical Records Request: Fill & Download for Free

Hi. I used to work in medical records for an orthopedic practice, and later, ran the same office when we converted to electronic medical records. Every doctor should be perfectly ok with their patient seeking a second opinion. You should be wary if any doctor discourages it, in fact.That being said, unless your current doc is in a very small practice, it's unlikely he or she will be informed about a medical records request. Most docs simply don't have the time or are not very interested in the minutiae of running the office.

The official answer is that the President needs to have his medical records at the White House Medical team's fingertips. Under HIPAA, copies of these records (minus psychotherapy notes) have to be sent at the request of the patient. This does not mean that the originals have to be given up (unless there is a warrant) but that copies have to be given. Summary of the HIPAA Privacy RuleUnder HIPAA, at least according to the news report, this appears to be a violation of the Privacy Rule and may be a criminal act which is subject to a $50,000 fine and a year in jail. The article I read stated that Trump's bodyguard took the original records because he could not get a copy due to the copy machine being out of order. Nothing was said about permission, etc. As far as i can read, there is no Presidential right to the original records. If a lawyer was present, he was not a very good one.As for Trumps' doctor, he is clearly in violation of the Privacy Rule and, judging from his public statements, not aware of the Rule or the consequences of revealing information about the President. A perfect FUBAR as far as I can see.

Oh boy where do I begin. Being a doctor in India, there are a lot of things I wish my patients would know-1.VIP status does not trump a medical emergency. Yes I heard you the first time that you are a prominent member of a political party. That does not, absolutely not, make your cough for the last month more urgent than an elderly gentleman I am evaluating for possible myocardial infarction. In the medical emergency those who can't wait due to the urgency of their illness come first. No exceptions. Triage is in place for a reason. No, I don't care if you call another prominent politician, or the media. I will do my job and duty first. And despite the rudeness, I will still get to your complaint and problem as soon as I'm able,to the best of my abilty.2. Saying I don't know what's wrong with you yet doesn’t mean I am not giving you my due attention. Nor does it necessarily mean your doctor is incompetent.I understand. The fear of the unknown is scary. But its as scary for your doctor too. If I'm still working up on your diagnosis I can only list the possibilities. The differential diagnosis. Until I know more, it would be wrong for me to worry you. Have patience. I’m on your team against the disease. The human body is complex. Sometimes it takes time.3.Each and every one of my patients matter to me as a doctor.I can understand how the patient who has psychosis worries you being in the same general ward. Or the one with handcuffs and police men posted near his bed. But their lives matter as much to me as an upstanding pillar of society such as a teacher. My priority will be decided by who needs my attention while on call. Not by the status, gender, profession, caste, previous deeds in life. Im your doctor I'm not here to judge. I'm here to do my best to treat.4.Please vaccinate your child. Self explainatory. Free vaccines provided by the government. Schedules available with every pediatrician and community medicine colleague of mine.5. As an allopathic practioner I really can't say how that particular ayurvedic or homeopathic medicine will work.Please understand we are taught allopathic medicine. And there are not enough trials available to give a good opinion about alternative medicine such as these. That being said, if you will ask me, I will give you an honest, personal opinion after clarifying the above, as to how my patients reacted in the past.So maybe thumbs up to the yoga and meditation? But not to sole use of ayurvedic herbs for diabetic control? No? Sadly, that's all I can, in good faith, say.6. I am human. I will make mistakes.I'm still learning. I will apologise if I make a mistake. We work in a team which helps minimise that possibility. Your best interests are mine too.The harm or loss to a patient I take personally. Death of someone more so. A lot of my colleagues and I face anguish and sleepless nights wondering what we did wrong. Taxing hours and lack of sleep and the skewed doctor- patient ratio doesn't help.I fear for my life in today's scenario. With reports of residents losing their eye or worse injuries. We do the best that is humanly possible. Please resort to courts if you have to, but spare us the physical abuse.Someday, it could be a doctor you know, who is harmed/killed.7. Please follow up when asked. And keep your medical records.This is for your benefit as much as mine. The government sector has me seeing so many patients everyday,that no matter how we bonded the last time you visited, I wont be able to recall the details of your illness beyond a point.I may not recognise you either. Don't take it personally. I request you. I wish to remember all my patients and mostly I do, but sometimes I forget. And to take a chance on your health by relying on just my memory seems frivolous and dangerous, don't you think?8. The blood tests I ordered will not drain you of all your blood. Neither will donating blood. I don't get any personal benefit by causing you pain. And I have donated blood myself.No, we can't use black market blood or animal blood for your patient.( I can't believe sometimes I get asked this by relatives of the patient after an hour of counselling them why blood is needed.)9. Diseases like diabetes or hypertension can be managed ; in most cases they can't be cured (excluding secondary causes) so please be compliant with your medicines.Just because now your Hba1c is perfect doesn't mean you can stop your regimen altogether. Just because your father feels better doesn't mean we can terminate the 6month ATT course for pulmonary Kochs in two months.The durations are there for a reason. My hope is that you will follow them.These are the ones I can think of now. Will add more later.
