Psych professionals are there to help you, not harm you.It's hard to be a teenager, because you haven't had enough experience with life to know what's really harmful in many areas, and which things seem bizarre to you, but are pretty much normal. Even if you've moved a lot, had a lot of changes, you are just beginning to reach adulthood, and it's hard to identify what's making you feel worse.The kind of screening a teen goes through partly depends on where it's done and who's doing it, and for what reason.I've admitted teenagers to psych hospitals, and a nurse's screening contains a lot of information but is not as detailed as the screenings other professionals may do.I'd been admitting people to the hospital for several years when I changed jobs to a residential treatment facility for teenagers only. On my first admission, I mistook the social worker (who was young) for the patient (who was a very butch girl with short hair). I was embarrassed to no end. They were both gracious about it. The patient, let's call her "Xiomara," became a favorite of mine, and I would ask her why she did things that she wouldn't explain to others. Most nurses tend to be sort of matter of fact, and easier to talk to than a lot of other staff.Our first interest is what problem brought you to the hospital (or to treatment). My old joke is "What brought you to the hospital," and most people said, "An ambulancc." That gave both of us a chance to laugh and relax a little.People rarely seek treatment unless they are having extremely painful problems in life. No diagnosis is formed on first assessment.We'll ask:What kind of problems are you having? How much have they changed your life?Do you have any impulse to hurt yourself? Have you acted on it? How many times? Do you have the feeling now/are you feeling safe with yourself?Has anything traumatic happened to you lately or during your life (with an explanation of our definition of trauma).Are you using drugs/alcohol? Do you need detox?Have you had any problems with the law?Do you live with your family? Tell us who lives in your household. How do people get along in your family?Do you have any impulses to be violent? Have you acted on them?Any history of physical illness?Teenagers usually come to a psych unit from the emergency room, with some basic history taken there. If the teenager doesn't volunteer the same information, we allude to it.What do you think we can do for you that would help you the most?Are you on any medications currently? What are they?Do you ever hear voices that other people don't, or have thoughts about strange behavior other people find odd?Most screeners/interviewers have lot of experience with truly horrible life experiences of their patients, if not in their own lives. Almost nothing shocks screeners, so you can be open. We've been desensitized, or we couldn't do our jobs. We are usually part of the treatment team that sees you the most, and we truly care about how we can make your life better.Psychologists and psychiatrists may have you complete standardized questionnaires for diagnoses, but not on the first screen.The questions about being violent, impulses to hurt yourself, hearing voices may seem odd if you don't have them, but many people having psych treatment do. It's absolutely ESSENTIAL to ask these things. That's why you get asked.Diagnosis is never as important as finding the tools to help someone feel better and function better.
