PDF Editor FAQ

What is the difference between mental status tests and neuropsychological tests in Alzheimer's diagnosis?

Mental Status examinations are performed by health care practitioners as part of neurological, psychological, psychiatric, or medical evaluations. These examinations are generally fairly brief in duration and limited in scope. For example, when I perform a neurological examination I examine the patient’s motor, sensory, cerebellar, reflexes, gait, and cranial nerve systems, and I perform a mental status examination. The mental status portion of the evaluation may be a formal examination and include tests such as the Montreal Cognitive Assessment, or the Folstein Mini-Mental State Examination. These examinations take between 5 and 8 minutes to administer. Sometimes a clock drawing test is administered. This generally takes 1–2 minutes. Alternatively, I may do a less formal and much briefer mental/cognitive assessment as part of the mental status examination.Neuropsychological tests are formal tests that are typically performed by neuropsychologists. The tests typically take 2–6 hours. They generally include the administration of several formal tests of cognition. The examiner also generally obtains a detailed history of the patient's medical, mental health, and neurological state. A review of neurodiagnostic studies such as CT, MRI, or PET of the brain, and blood tests may also be done. The neuropsychologist provides scoring for the various tests that were administered as well as interpretation of the results. Generally, the neuropsychologist makes recommendations for further testing, or in some cases for follow-up testing.

In a psychological evaluation, when would they ask a patient to draw a clock? How are the results assessed?

You are referring to the Mini-Mental Status Examination (“Folstein Test”) or some variation of it, which is a quick and dirty way for a clinician to not only screen for neuropsychiatric problems, but also to follow the progression of such diseases themselves. It is, and has never claimed to be, a diagnostic tool.Let me breakdown the clock part. What does it take to draw one? First, memory. You have to remember what it looks like and which numbers go where. Obviously, if cannot you have a problem.Second, concentration. Think of someone with untreated ADHD or ADD, for example. Your behaviors are also being monitored by the practitioner.Last, it tests for motor skills. Even fine tremors will be evident.Other parts of the exam test these faculties too, so the final assessment takes all results into consideration.
