What is the most brutal injury you've seen at an ER?

A few years ago, my hospital developed or adopted a system that would involve assigning two medical personnel to be on standby, should a catastrophe overwhelm a department, The hospital’s house supervisor made these daily assignments. Two people from any given unit would be on standby, thereby, leaving their assignments to attend to the emergency. It was called a “Code Dragon”. We no longer have a “Code Dragon”…it has been restructured and is now known as a “Rapid Response”, which is by far a stronger team involving more medical personnel, such as, RTs, transport team, physicians, house supervisor, chaplains…It just so happened one night, that a clinical coordinator from the PICU and I were called to the ER to assist with an MVA or motor vehicle accident, where a mom and her four children were involved. The mom was transferred to an adult hospital after sustaining some minor injuries and was released shortly afterward.In fact, it was the very first “Code Dragon” ever called…and this PI nurse and I were the first responders to assist the ER team. When we got to the ER, it was like a complete disorganized mess, All this unfolding before our eyes. The PI nurse and I stood helplessly along side a wall as if watching atoms and molecules display disorganized energy of motion…quickly the team began assuming their roles in this trauma situation, coming together calmly while order was being established. It was absolutely impressive to watch this team come together like clock work.The cubed rooms expanded as the partitions were drawn back expanding the space, thereby, allowing three siblings to be triaged and medically treated in this same vicinity. All were badly shakened up and very scared. There were a couple of deaths involved in the accident, whereby, another vehicle was involved. Four siblings, a 15yo, a 14yo, a 10yo, and a 6 yo, were brought by ground transportation to our facility. Three of the four sustained substantial, but non-life threatening injuries. Two of the three were taken to the OR. However, crying and shook up, all three were seen alert and oriented to where they were.In the very next room laid the body of their 14 yo brother. The mother arrived sometime later and we witnessed this family learn of the death of a son and brother. It was a horrific scene, one of the worst I have ever witnessed. There is nothing like the screams of a mother losing a child. No medical breakthrough in all of history has the capacity as of yet..to fix a broken heart. I witnessed the oldest being told that her brother didn’t make it. Pretty sure none of our eyes were dry.Since this was not our department, although, I have been floated to the ER in the past several times… we didn’t really know where all the supplies needed.. were kept. Especially under pressure, we both were out of our element initially. The ER Charge was way too busy to stop and assign us tasks. So, as we remained, I heard someone yell, “we need fluids and tubing primed here”....”well I can do that!” Then hearing “we need an IV established here”…. The PI nurse said “I can do that!” Soon, there after, we found ourself part of the team actively assisting. It was tragic, yet amazing to watch this team bring about such a powerful force of organization.ER Nurses and Doctors are an amazing team of professionals, whose critical thinking and communication skills, respect, and confidence, while calmly and level-headedly working together under intense situations saving lives, is none the less simply amazing!
