Henry Ford had a saying, “If you think you can or if you think you can’t, you’re right.” (or something like that). If you expect therapy to fail, then that’s probably what you’re likely to get, but not always. Depends on what you want to change, whether you’re capable of changing it, how you define success and failure, and who you get as a therapist.Lets start with the first two, the what you want to change and whether or not you’re even capable of changing. Some people are not going to be able to achieve their goals no matter how much they want to because they’re not capable of it. For example, I’d like to be able to dunk a basketball, but I’m 60, 5′10″ and can’t jump. So, no matter how much I want to dunk a basketball, even if I have the best basketball coaches in the world, and plenty of time to practice, without a trampoline, or a lower hoop, I’m NEVER going to dunk a basketball.There are some disorders - ADHD, for example - that aren’t really treatable. If you have ADHD, you have to learn how to live with it. Don’t work in a busy office where there are a million distractions when you have to focus and concentrate. Find a career where having ADHD is an asset (like park ranger). If you have a personality disorder, there isn’t a therapist in the world who can ‘cure’ you. But that doesn’t mean your disorder is not treatable. Dialectical behavior therapy is an effective treatment for borderline personality disorder, for example. Or consider substance use disorder. Once a patient develops addictive disease, they always have addictive disease. They can manage the disease with a variety of coping strategies in much the same way that a patient with diabetes or heart disease uses diet, exercise and medication to stay as healthy as possible, despite the disease.The psychiatrist Renata mentioned has a style. He’s not going to spend his time trying to help someone change if they don’t really want to change. If you get him, and you don’t want to change, he’s not going to treat you, and you’re not going to change.On the hand, we all have a natural resistance to change. (There’s another saying: ‘people prefer the devil they know, to the devil they don’t.) If change were easy, you wouldn’t need the therapist. You would decide to do something different, and like the famous shoe company says, you’d “just do it.” A psychiatrist named Milton Erickson believed that the therapist’s job is to find the key that unlocks the patient, so that the patient can live a more meaningful life. For most patients, their conscious ‘decision’ that psychotherapy will fail can be ‘bypassed’ by therapeutic intervention. If the therapy fails, it’s not because the patient didn’t believe that it would, but because the therapist couldn’t find the right intervention in the time they had to work. Sometimes, finding the right intervention is beyond the skill level of the therapist, sometimes the therapist runs out of time and sometimes the work requires more therapeutic energy than the therapist has to give, but it doesn’t matter what the patient believes if the therapist is skillful enough, and has the time and energy, to bypass the patient’s resistance. Erickson’s approach and anecdotal success as a therapist is well documented, but I will share one favorite story.Erickson lived in Arizona because of his health (he had polio as a child). He was going to a conference in Milwaukee, and a doctor friend asked him to look in on his mother, who he suspected was depressed, to see if he could do anything. Erickson agreed, and visited the woman after the conference. She lived in a regal, old, three-story Brownstone. The shades were drawn, the house was dark. They had a nice chat over some tea, and it was clear that the woman was depressed. Before he left, she asked Erickson if he would like to see her orchids. He said yes. The woman led Erickson to a greenhouse on the roof of the Brownstone in which she grew the most beautiful orchids. Erickson admired them, then soundly chastised the woman for her selfishness. He told her, “You should be ashamed of yourself! A good Christian woman like you! Here you are growing these beautiful orchids and hoarding them all to yourself, when there are people in hospitals and nursing homes all over the city with no one to visit them or brighten their day.” She threw him out. Erickson went home.Years later, Erickson’s friend mailed him a clipping from the Milwaukee newspaper. The headline read: “Orchid lady dies. Mourned by thousands.”I had a client who had been in and out of therapy for 30 years, suffering from depression. He came to therapy at the agency I was working at and wound up transferred to me (after he freaked out the novice therapist he was originally assigned). He told me that therapy was pretty much a bunch of hooey, therapists were all quacks (many of whom used their sessions to tell him about their own problems), and he didn’t expect much of anything useful to happen. But he felt like he had to try. He was in his 60’s, had a son approaching 40, and told me that while he would never kill himself because it would hurt his son, if he simply didn’t wake up one morning, that would be alright with him. He believed that the only way he would get better would be by engaging in ‘Imago’ dialogue discussing our respective childhood wounds with his therapist, and by ‘healing the healer’, would himself be healed, but didn’t think he’d ever find a therapist willing to go there with him.We divided our time between him talking and me listening and reflecting, and me telling him stories that I thought might be useful (i.e., therapeutic). He regularly complained that my stories were a waste of our time, but he came faithfully to our sessions, largely because he had to in order to see the agency psychiatrist, who prescribed anti-depressants. Somewhere in the course of our work, he told me about a helpful “body spirit” that visited him from time to time. I encouraged him to listen to and abide by the wisdom of his body spirit. Whether it was the medication, the body spirit or our ‘Imago’ dialogues, his depression lifted. In the beginning of our work together, he had consistently rated himself at ‘0’ or slightly higher (on a scale from 0=worst to 10=best) in all life domains. By the time I left the agency, he was rating himself at 9 or better and only coming once every month. I think he wanted to feel better, but didn’t believe that it was possible - until he started to feel better.I’ve also worked with plenty of adolescents who were dragged to therapy kicking and screaming by their parents. Most of the time, each wanted the other to change, and neither was very interested in changing themselves. Some of them wound up changing in a positive direction anyway, but I sometimes ran out of time before I found the right key. Perhaps another therapist would have been more effective with some of them; I’ll probably never know to what extent the therapy was helpful.Whatever you decide, I hope you find what you’re looking for.
