It was 3 pm on a rainy day. I was sipping a cup of tea in the hospital canteen and checking out the cricket match scores on my phone.Just then the speakers started blaring 'Code, Blue, Code Blue, ICU'. The dreaded words hit me. I immediately sprinted to the ICU on the third floor, I used the stairs and didn't wait for the lift.While running, I was thinking which patient was it, there were four of them in ICU and all of them were stable when I last saw them. I reached the ICU door and flung my shoes out and didn't bother to scrub my hands which is the usual practice.I saw my registrar doing chest compression on the patient in the second bed. The nurse was bagging the patient with an ambu bag to push air into the lungs.The patient was a 15-year-old girl with blood cancer (Acute Myeloid Leukemia) whose disease had relapsed after successful treatment. She had come in sick and had a serious infection in her blood. We wanted to control her infection and start chemotherapy again. Her chances of cure were slim but we were hopeful. She wanted to be a dancer. She was brave. Her blood pressure was low and she was on medicines to keep them up.As I reached near her, I saw blood was gushing from her mouth. She had a massive pulmonary hemorrhage (bleeding) in her lungs and had suddenly collapsed.I asked the registrar to stop the chest compression for few seconds to check the heart rhythm on the monitor; it showed ventricular fibrillation (VF), an uncontrolled chaotic fluttering of heart. Meanwhile more doctors and nurses had joined in.The advanced cardiac life support (ACLS) resuscitation protocol was initiated. The patient was connected to the defibrillator, my registrar shouted 'I am clear, are you clear, all clear' and delivered the electric shock to the patient's chest to bring back the heart rhythm to normal. The girl's body jumped a few centimeters above the bed. The CPR was resumed.The anesthetist was struggling to intubate (put a tube in to the wind pipe to assist breathing) as the mouth was filled with blood. He somehow managed to put the endotracheal tube (ET) in and continued bagging. Chest compressions were continued and 2 minutes later the monitor still showed VF and the whole sequence of shock with the defibrillator was repeated.Blood was gushing out of the ET tube and fluids and blood products were being rushed into the patient. There was no pulse or blood pressure recordable.She lay motionless, pale, limp and her chest exposed. There was blood everywhere. Her hands and feet were cold and her pupils were fixed and dilated. On shining the torch in her eyes the pupils didn't react and the cornea had become cloudy. The heart monitor showed a flat line and multiple doses of adrenaline were pushed inside her.It was like her soul was standing beside me and watching what we were doing to her body. It was about 60 minutes of resuscitation now and I felt her telling me 'let go, doctor, your team tried its best'. With a heavy heart I asked the CPR to be stopped; there was no way she could recover now, she was dead.Everyone had their heads low. The doctors and nurses from other wards went back to work. I was standing next to her holding her hand and touching her forehead. She still looked alive, it was like she was just sleeping. We had just spoken 2 hours ago, she was unwell but was telling me about her birthday plans and now on the white hospital sheet stained with blood lay her lifeless body.Her parents rushed in sensing that things were bad. Her mother saw her body and gave a shrill cry and collapsed to the ground. The dad just bent down, his knees buckled and he hugged his daughter. I was standing holding her hand. The dad looked at me and his eyes were pleading with me ‘please bring back my daughter’. I couldn't meet his eyes . I tried my best to console them but it was futile.I left the family alone and went and washed my blood-stained hands. I started completing the documentation process. Half an hour later her body was wheeled out in front of me. Draped in white cloth secured tightly by the cotton ropes was a thin tall figure and slowly it moved into the distance with her wailing mother and shocked father walking behind her.I have gone through such situations numerous times in my medical career. From a child, to an adult and elderly I have seen them all dying in front of my eyes and in my arms. Some were expected and some were unexpected. But it was all the same. Sometimes you and the family are relieved that the end has come as the patient had been through lot of pain and suffering, yet other times it comes as a bolt out of the blue.I never get excited about birthdays, either mine or my family members'. It is just another day for me. My wife used to get upset that I was being insensitive.
