For example, gun shot wounds must be reported.This is what the American Medical Association says on the subject:Safeguarding patient confidences also is subject to certain exceptions that are ethically and legally justified because of overriding social considerations. If there is a reasonable probability that a patient will inflict serious bodily harm on another person, for example, the physician should take precautions to protect the intended victim and notify law enforcement authorities. Communicable diseases and gunshot and knife wounds should be reported as required by applicable statutes or ordinances. Thus, the physician's duty of confidentiality at times must give way to a stronger countervailing societal interest. [1]On a personal note, though I'm not a doctor, I am required to maintain patient confidentiality under both federal law (Health Insurance Portability and Accountability Act) and state laws/regulations relating to my EMT certification. Even so, I have the same considerations as a doctor. If a patient tells me they're going to kill someone (besides me; I've been threatened by patients many times), you better believe I'm going to share that information.I've actually been considered a "mandated reporter" in several jobs, including as a childcare worker at an after school program, as a summer camp counselor, and now as an EMT. Of those three jobs, only the third was a professional certification requiring confidentiality, but if I ever have any reason to suspect child abuse or maltreatment, I'm required to report it. To become an EMT, I had to learn about the Ryan White Care Act, which allows EMS personnel to find out when they may have been exposed to life threatening diseases while providing care. I wouldn't consider that breaking confidentiality since they would've already been involved in the patient's care, but this information sharing would be after the fact and so may be worth mentioning.[1] Patient Confidentiality

There’s nothing wrong with telling your therapist names. Even if it’s a company-hired therapist at work, the most they can do is be amused by work drama amongst themself.There’s special doctor-patient privilege in place to prevent your doctor from spreading personal information, whether it be about your personal health (sensitive information about STI’s and STD’s aren’t exactly things people want floating around) nor are they allowed to talk about your personal life. No matter how much someone asks, they’re by law not allowed to divulge information.Usually there’s a clause in this agreement, where the doctor in question can send information to other health-care facilities with your approval, or in the case of an emergency, can be sent without your approval. The same goes for if you tell them something in confidence; the news about Rachel’s fling with your best friend Ross can’t be shared, but if you’re showing all the red flags about suicide, then the doctor in question can admit you into an emergency care center to keep you from doing harm to yourself. If you talk about Phoebe’s mom showing flags of suicide, I believe the most they can do is just advise about what you can do to help, but therapists themselves are sometimes not allowed to intervene in someone’s personal life just because “my client x said so”. It’ll vary from state to state, and you’ll be able to verify that based on the confidentiality agreements you signed.
