As it is difficult to discuss all the perspective of Indian health system in a single article, here I have given the basic outline of initiatives taken by the Government of India in perspective of Non Communicable diseases. At the end of this article you will have a basic idea about Indian health system and its service delivery in perspective of Non Communicable diseases. Its might be bit long, so bear with me.Non-Communicable Diseases (NCDs) are the great risks of today’s domain of health. The risk is considered greater because often Non-Communicable Diseases do not show any sign and symptoms in the first hand. This characteristic of NCDs is the main reason people fall in the trap of NCDs because they often neglect this in the first place. To combat NCDs is high public health challenge due to high morbidity and mortality associated with these diseases. In particular, four diseases, viz., CVD, Diabetes, Cancer and Chronic Obstructive Pulmonary Disease, account for large morbidity and mortality due to NCDs.Early detection is an important strategy to prevent and control NCDs. It is common knowledge that timely detection, diagnosis and treatment lead to reduction in morbidity and mortality arising from common NCDs.I can’t discuss in detail about a particular disease in a single article. Here I will write about the schemes and facilities available in India so that you grow a basic knowledge about it. It will help you to avail the service at any Government health centre which is open to all. Later I will write on every diseases covered in the programme in detail so that you could be benefited with it.Lets know the programmes of Non Communicable Diseases run by Government of India one by one.National Programme for the Prevention & Control of Deafness (NPPCD)Hearing loss is the most common sensory deficit in humans today. As per WHO estimates in India, there are approximately 63 million people, who are suffering from Significant Auditory Impairment.The services Delivery:1. Early detection of hearing impairment and deafness. (Screening & OPD)2. Treatment of the diseases. (If necessary then operation is carried out).3. Management of hearing and speech impaired cases and rehabilitation.The NPPCD programme is being executed from Sub-center level to tertiary level. Large scale direct benefit of various services like prevention, early identification, treatment, referral, rehabilitation etc. for hearing impairment and deafness is being carried out in all the Sub-centres, Primary Health Centres (PHC), Block Primary Health Centre (BPHC)/Rural Hospital (RH), Sub-Divisional Hospital, District Hospital. The ENT surgeon of Sub-divisional Hospital & District Hospital is there for the necessary service to the people. There is an Audiologist, an Audio-metric Assistant and an Instructor to cater the hearing issues of common people.Health camp for the detection of hearing impairment also been carried out in regular interval as an opportunistic intervention.National Programme for Control of Blindness & Visual Impairment (NPCB&VI)Do you have any of these symptoms?1. Severe, sudden eye pain.2. Recurrent pain in or around the eye.3. Hazy, blurred, or double vision.4. Seeing flashes of light or sudden bright floating spots.5. Seeing rainbows or halos around lights.6. Seeing floating “spider webs”7. Seeing a “curtain coming down” over one eye.If yes, then you are at great risk of having eye diseases.With ageing the eye diseases like Refractive Errors, Cataract, Diabetic Retinopathy, Glaucoma etc started to happen. If you diabetic then the risk factor increases even more.But don’t worry.There is a health mechanism throughout India at your door step to address your eye problems.What are the services you can avail under National Programme for Control of Blindness & Visual Impairment?1. If you are above 45 years of age then get screened at your nearest health facility immediately. If you have presbyopia then you will be provided free spectacles after treatment.2. Free spectacles also provided for students of secondary school after screening of reactive errors.In any kind of eye diseases like Cataract, Diabetic Retinopathy, Glaucoma, Corneal Blindness, Low Vision etc. please contact to the nearest Vision centre of yours. All the service deliveries are provided solely by Govt. of India and its always free.Difference between Normal vision and Glaucoma visionThere are Eye Surgeons in Darjeeling District Hospital, Siliguri District Hospital and Kurseong Sub-divisional Hospital where the eye operation is being executed and spectacles are given free from Government.Mental Health ProgrammeMental Health is an important issue now days. A healthy mind reflects the all over wellbeing of a person mentally and physically. As the problems is not always visible in empty eye, its need an extra care and empathy towards the patients.What are services you can avail under Mental Health programme?1. The services starts from the grass root level. All you have to do is to contact at nearest sub-centre and inform the health worker about your mental health condition. There is a swift referral system mechanism to address the problem. After analyzing the gravity of your problems you will be referred to necessary strata of health system like Rural Hospital/Block Primary Health Centre, Sub-divisional Hospital or District Hospital.2. There is already trained Medical Officers and Staff Nurses at Rural Hospital/Block Primary Health Centre and Sub-divisional Hospital or District Hospital. They are trained by qualified psychiatrist for early identification and management of mental disorders.Feel free to discuss any issues to the medical officers and staff nurses at your nearest Health facility.National Tobacco Control Progarmme (NTCP)There are almost 267 million tobacco users in India. Each year tobacco use kills about one million Indians. Heart disease and stroke are the commonest ways by which tobacco kills people. 31st May of each year is being celebrated as No Tobacco Day to create awareness regarding the fatal effect of tobacco consume.District Tobacco Control Cell of Darjeeling district is working on 3 components of the program.(A) Awareness (B) Law Enforcement & (C) Tobacco CessationA) Awareness:1. Training of health workers, school teachers, police and enforcement officers is being held time to time to create awareness regarding the ill effects of tobacco and the COTPA-03 act.2. Conducting IEC activities among common people/civilian, shop keepers with the warning of tobacco consumption and sell.3. School and College programmes are being carried out through different attractive activities involving both teachers and students.B) Law Enforcement:1. To implement the Cigarette and Other Tobacco Products Acts 2003 (COTPA) a number of Law enforcement programme carrying out time to time in coordination with other line department such as education, police etc.2. The shop keepers who sells tobacco products within the radius of 100 yards of any school and college are being fined up to Rs.200 and warned.3. The people who smoke in public place also charged up to Rs.200 for violating the law.C) Tobacco Cessation:1. There is a Tobacco Cessation Centre functioning in most of the District Hospital and is open on all working day. A Medical Officer and a Clinical Psychologist are there at the the Tobacco Cessation Centre and taking care of all the needs of medication and counselling.2. Clients who are addicted to any form of tobacco are given proper counselling here. The psychologist gives counselling in several sittings if needed and follow up the development of clients.3. Those who are highly addicted to it are given medication in the form of Nicotine chew-able gum or Nicotine Patches under the supervision of nodal medical officer of Tobacco CessationCigarette and Other Tobacco Products Act-2003In order to protect the public from the adverse and harmful effects of tobacco use and second hand smoke (SHS) and to discourage the consumption of tobacco, the Government of India enacted the Tobacco Control law titled “Cigarettes and other Tobacco Products (Prohibition of Advertisement and Regulation of Trade and Commerce, Production, Supply and Distribution) Act, (COTPA) 2003”.According to COTPA-2003:1. No one is allowed to smoke in public places. (Maximum fine Rs.200/-)2. Tobacco products cannot be advertised3. Tobacco products are not allowed to sold near educational institutions (100 yards from the outer boundary of educational institutions)4. Tobacco packets should carry warnings about the risks of using tobaccoQuit Tobacco today for a healthy futureNational Oral Health Programme (NOHP)“If the oral health is neglected for a long time with a habit of substant abuse then the chance of having disease like oral cancer increases.”National Oral Health Programme is successfully running to improve the determinants of oral health and to reduce morbidity from oral diseases. It could be availed by the common people from primary level to tertiary level.Oral health is a state of being free from chronic mouth and facial pain, oral and throat cancer, oral sores, periodontal (gum) disease, tooth decay and tooth loss, and other diseases and disorders that affect the oral cavity.Do you know, Oral diseases are linked to chronic obstructive lung diseases and diabetes also?Aged people are more vulnerable to the oral health problem. So, don’t ignore your oral health.What you should do?If you have any problem regarding oral health then go to your nearest health centre. There is qualified Dental Surgeon in every Block Primary Health Centre. Dental Hygienist&Dental Assistants are available at Medical Colleges.The Oral Health affects more than what we thinkWhat are the services available?1. Overall assessment of oral health conditions2. Root canal treatment3. Oral Prophylaxis4. Patient education regarding oral hygiene maintenance etc.What you should do to maintain your good Oral health.1. Brush your teeth thoroughly twice a day and floss daily between the teeth to remove dental plaque.2. Visit your nearest health centre often, even if you have no natural teeth or have dentures.3. Do not use any tobacco products. If you smoke, quit.4. Limit alcoholic drinks.5. If you have diabetes, work to maintain control of the disease. This will decrease risk for other complications, including gum disease. Treating gum disease may help lower your blood sugar level.6. Drink plenty of water, chew sugarless gum, and avoid tobacco products and alcohol.Don’t ignore your oral health.National Programme for Prevention and Control of Cancer, Diabetes, Cardiovascular diseases and Stroke (NPCDCS)The objective of NPCDCS Programme:· Prevent and control common NCDs through behavior and life style changes· Provide early diagnosis and management of common NCDs· Build capacity at various levels of health care for prevention, diagnosis and treatment of common NCDs· Train human resource within the public health setup viz. Doctors, paramedics and nursing staff to cope with the increasing burden of NCDs,· Establish and develop capacity for palliative & rehabilitative care.1. Do you have a family history of diabetes and/or hypertension?2. Are you overweight?3. Are you more than 30 years old?4. Do you drink alcohol and/or use tobacco?If yes, then you are at great risk of having the diseases like Cancer, Diabetes, Cardiovascular diseases and Stroke.Don't ignore, immediately get your Blood Sugar checked and measure your blood pressure at your nearest Health Centre.It is not necessary that you may have any signs or symptoms. If you have any kind of NCD related problem then you can approach at sub center at your village.Here you can do Diabetes Test, measure your Blood Pressure and could be screened for other Non-Communicable Diseases like cancer.Cancer is uncontrolled division of cells or abnormal increase in the number of cells. This destroys the normal function and division of cells.Types of Cancer:Common Cancers in males: Mouth, Lungs and StomachCommon Cancers in Females: Cervix, Breast, MouthDon’t ignore the early symptoms. Be yourself screened at your nearest sub-center.National Programme for Palliative Care also running at District Hospital & by trained specialized Medical Officer.If you your age is 30 or above 30 yearsWhat you should do?· Check your blood pressure and blood sugar at your nearest health facility. The Community Health Officer and other health staffs are available all across Darjeeling district at your service.· Abide by the instruction of health staffs and change your lifestyle and diet for healthy life.· Do physical activity/exercise or walk at least 30 minutes a day for prolonged active life.· Take seasonal fruits and vegetables of every available color. Avoid junk food.· Avoid tobacco or alcohol or any kind of substance abuse.Chronic Obstructive Pulmonary Disease (COPD)& Asthma“At one time, COPD was more common in men, but because of increased tobacco use among women in high-income countries and the higher risk of exposure to indoor air pollution (such as biomass fuel used for cooking and heating) in low-income countries, the disease now affects men and women almost equally.”According to WHO estimates, 65 million people have moderate to severe chronic obstructive pulmonary disease (COPD). More than 3 million people died of COPD in 2005, which corresponds to 5% of all deaths globally. Most of the information available on COPD prevalence, morbidity and mortality comes from high-income countries.It is known that almost 90% of COPD deaths occur in low- and middle-income countries. This is the 2nd leading cause of death among Non-Communicable Diseases in India.At one time, COPD was more common in men, but because of increased tobacco use among women in high-income countries and the higher risk of exposure to indoor air pollution (such as biomass fuel used for cooking and heating) in low-income countries, the disease now affects men and women almost equally.In 2002 COPD was the fifth leading cause of death. Total deaths from COPD are projected to increase by more than 30% in the next 10 years unless urgent action is taken to reduce the underlying risk factors, especially tobacco use. Estimates show that COPD becomes in 2030 the third leading cause of death worldwide.Causes of COPD:· The primary cause of chronic obstructive pulmonary disease (COPD) is tobacco smoke (including second-hand or passive exposure).· Other risk factors for COPD include occupational dusts and chemicals (such as vapors, irritants, and fumes) and frequent lower respiratory infections during childhood.This programme is a specialized one under the umbrella of NPCDCS programme. The screening, referral system and treatment is already available from the low strata of health mechanism. It starts from the Sub-center level, PHC, BPHC level to the Sub-divisional Hospital and District Hospital.Please contact at your nearest health centre if you have any issues regarding COPD & Asthma and make your life healthy and happy.Chronic Kidney Disease (CKD)“Early detection and treatment can often keep chronic kidney disease from getting worse. National Iodine Deficiency Disorders control Programme (NIDDCP) is an attempt to address the various aspects of the issues in India.The deficiency of iodine results in so many abnormalities like depressed mood, dry skin, fatigue, increased sensitivity to cold, Menstrual cycle abnormalities, Thickening of the skin etc.The most common visible sign of deficiency of iodine is Goiter (enlargement of the thyroid, creating a mass in the throat or neck). Apart from all these things miscarriage or still birth is common in Iodine deficiency. It affects the post-birth developmental issue like an iodine deficient person grows low IQ and often slows down the cognitive development.Components of the Progarmme:· Extensive IEC at sellers’ points is being carried out for wide publicity and create awareness for selling of iodized salt at shop in Darjeeling district.· Salt samples are collected from shops time to time and sent to the laboratory for volumetric test which detect the actual quantity of iodine in salt.· ASHAs are doing the iodine test in every household and AWCs. ASHA carries iodine kit provided to them from the Government and they go to every household, take sample and test by using the iodine kit on the spot and counsel the people accordingly.Be aware and spread awareness.Always take iodized salt.Don’t put your salt container open for long time.A simple steps like intake of iodized salt will make your future healthy and happy.Thalassemia Control ProgrammeThalassemia is a disease of the blood in which there is increased destruction (hemolysis) of the red cells. There are various severities of the disease from thalassemia major-the severe form in which the patient needs regular blood transfusions in order to survive, to thalassemia minor an asymptomatic carrier state with patients of thalassemia intermedia in between.This is an inherited (can be passed on to the children from parents) blood disorderRemember, a simple blood test is needed to detect whether you are thalassemia career or not.
