In the 70s, eight sane, average, working people sought to gain admission to psychiatric hospitals.They kept their lies to a minimum; they provided their psychiatrists with accurate details about their lives, relationships and jobs (except for those who were psychologists themselves, as they didn’t want differential treatment). Their complaints contained only a single fabrication: The pseudopatients claimed they heard vague voices saying “empty”, “hollow” or “thud”.As auditory hallucinations go, this is a pretty tame one. Aside from the words being eerie as heck, there’s nothing necessarily upsetting or threatening about them. By modern standards, you’d probably be considered at mild end of the psychosis spectrum.Nevertheless, the eight patients were accepted into a total of 12 hospitals, from run-down, underfunded urban facilities to expensive, acclaimed medical centres. Each person was randomly allocated to a hospital, and they were admitted.From here on out, they acted completely normally. Even their reports of auditory hallucinations stopped. The actors stopped being actors and instead went about their healthy, mentally stable business within the ward.This was the whole reason they had entered the hospitals. They were all participants in an experiment designed by David Rosenhan, who wanted to answer one question: can we tell sanity from insanity?According to Rosenhan’s report: no.The seven participants who entered public hospitals were diagnosed with schizophrenia, while the one participant who entered a private hospital was diagnosed with manic-depressive psychosis. None of them were ever identified as impostors by the staff, although they were recognised by 30% of the other patients, who guessed they were researchers or journalists collecting information on the hospital. The participants certainly made no secret of acting like researchers; they openly took notes on the hospital and its patients. Some staff seemed to interpret this as a sign of mental ill-health, with one nurse labelling this “writing behaviour”, and considering it further evidence of the participant’s psychosis.The stay was boring, dehumanising and did little to actually support patients. The participants reported that the staff were generally well-meaning, but often invaded their privacy, watching them while they used the toilet, and objectified them, discussing patients openly as if the patients weren’t even there. Doctors spent minimal time with patients — an average of 6.8 minutes a day — and medicalised the most mundane of behaviours.Some staff members were verbally and physically abusive.The participants asked to be released, telling their psychiatrists that they were fine, and no longer had any symptoms. Most were released after “admitting” they were mentally ill, and agreeing to take antipsychotic medications, which they promptly flushed down the toilets. A lawyer was called for those who were never going to be allowed to leave voluntarily.The length of their stays ranged from 7 to 52 days, which, considering they all entered with identical symptoms, seems like a drastic difference. Upon leaving, the participants were diagnosed with schizophrenia “in remission”, a label that would stay with a real patient forever, and maintain the stigma even after so-called successful treatment.If sanity and insanity exist, how shall we know them?— Rosenhan, 1973, On Being Sane in Insane Places.[1][1][1][1]So, apparently, there’s no way to convince a psychiatric hospital of your sanity. Rosenhan argued that the diagnostic system was so flawed that psychiatrists couldn’t tell the difference between the sane and insane, the healthy and the helpless. We locked people up, took away their liberty, treated them as inhuman, all based on nothing. It was a wake up call for psychology — and it was an embarrassment.But it was also the beginning of an academic dogfight. This study is drowning in controversy, even as it approaches its 50th anniversary. According to some, Rosenhan concealed the accounts of participants who had positive experiences, just to create a juicier narrative. Some question why Rosenhan never published more works on this data — was it because Rosenhan knew it couldn’t stand up to scrutiny? One author claimed that she could only find evidence for two of the participants — one being Rosenhan himself, and one being a positive account — and the others’ records were untraceable.[2][2][2][2]Personally, I think it’s feasible that everything could have gone down exactly as Rosenhan suggested, given the state of mental health care in the 70s; psychology was still young, and no one had anywhere near as much knowledge or sensitivity as we do now.But I do question Rosenhan’s conclusions. The participants told psychiatrists that they were mentally ill and wanted to be admitted to the hospital for treatment, and it’s hardly surprising that the staff took them at their word. Mental health is a largely subjective experience, and it makes sense to rely on people’s subjective reports to diagnose them. As Seymore S. Kety (whoever he is) put it:If I were to drink a quart of blood and, concealing what I had done, come to the emergency room of any hospital vomiting blood, the behavior of the staff would be quite predictable. If they labeled and treated me as having a bleeding peptic ulcer, I doubt that I could argue convincingly that medical science does not know how to diagnose that condition.[3][3][3][3]The terrible conditions and baseless stay times, though… Yeah, that’s pretty bad.But I’d like to think psychiatric hospitals, and psychiatric diagnoses, are a lot better now. A recent study has reported high inter-rater reliability for the diagnosis of psychosis and depression,[4][4][4][4] and 71% of UK mental health hospitals have been rated as good or outstanding[5][5][5][5] . What struck me was the similarity in the phrases they used to appeal for donations.More than 70% of such NGOs are fake and they operate underground from residential areas. When you get a call from them, they also share a patient registration document which makes you think that the case is real.What actually they do is “illegally buy” papers from the hospitals of patients who were operated or deceased and modify them. There are multiple ways to counter check whether the call you got was real fake.Check whether the NGO is registered?
