Doctors, what is the rarest condition you have diagnosed and how did you come to that conclusion?

Scrapie, a disease of sheep and goats, is the prototypical prion disease. Mad Cow Disease, Kuru and Creutzfeld-Jakob disease are other examples. Prion diseases are infectious, fatal neurological diseases caused not by an organism, but by a mis-folded protein which recruits chemically similar proteins by remolding them into the infectious form. These re-folded forms of otherwise normal proteins cause horrible diseases in the infected host. It was in vet school where I first heard about prions (then a controversial theory for what were previously known as “slow viruses”). I found them fascinating and terrifying: there is no treatment and they are inevitably fatal. Fortunately, we were told, they are exceedingly rare.The veterinarian who taught us about prion diseases worked for the state department of agriculture, administering the scrapie eradication program. He described the eradication program as: “kill all the sheep, burn down the buildings and roll up the wire.” He also showed us a video he had taken of a peculiar reaction he had noted in affected sheep: when you scratched them over the withers (shoulder blades) they extended the head and neck and made a peculiar nibbling motion with their lips, almost like they were appreciating you scratching a place they couldn’t reach.When I started my first job as a veterinarian, my wife and I bought a house on 5 acres with lots of overgrown grass, a real fire hazard. Fully embracing the suburban rancher lifestyle, we bought 5 pregnant ewes from our friend Tipton. The next year, I was thrilled to be able to borrow a ram from one of my clients, who happened to have one of the most elite flocks of Suffolk sheep in North America. I think you can see where this is headed. This ram started to lose weight, scratch itself on any available surface and act strangely. When I pinched the withers, he nibbled. Oh no, what a dilemma. A diagnosis of scrapie in his flock would be costly to my client, but he had loaned me the ram as an act of generosity. I consulted with my mentor at the vet school and she counseled that it would better if the owner dealt with the problem, since the only way to diagnose scrapie at that time was through a post-mortem examination of brain tissue. I couldn’t kill a ram I didn’t own.I called my client, told him of my suspicion, strongly urged him to contact the state vet and gave him a copy of an expensive veterinary textbook which had the most current information on scrapie. I thanked him for his generosity. I hoped he would do the right thing.Three years later, having returned to the vet school to do advanced training in an internal medicine residency, I was at the local auction yard dropping off some of my surplus lambs when I noticed some tall, skinny Suffolk sheep in the sale pen. The pen card confirmed that they had been consigned by my former client. I leaned over the pen and scratched the withers. Nibbling. I called the state vet and told him of the suspicious sheep at the auction yard. I never heard directly what happened but noticed a few years later that my former client was no longer in the sheep business.I may be the only private practice veterinarian in California who has diagnosed this rare disease in three different flocks. The most poignant was in a 4-H project flock owned by a conscientious teenager who was counting on selling his prize sheep at a premium to finance his college education. The diagnosis of scrapie was emotionally and financially devastating to him. The state vet “depopulated” his flock and he received a nominal indemnity payment. Heartwarmingly, our community rallied to his cause and at the annual Junior Livestock Auction, his (now only symbolic) flock received the highest price and he got a nice check. His peers got a good lesson in honesty and integrity.

How do I get a driving licence in Mumbai?

It has become much easier now. Here is my experience of the entire process. I did not go through any agent. I just wanted to see if I get the license without any agent. It was not about spending extra bucks, but I just wanted to enjoy the adventure.Please note that I applied through Mumbai Wadala RTO and this is just my experience. Your RTO may be a bit different. You may experience something different even if you are applying through same RTO. I believe the processes and their applications are always evolving and you may get a better or worse experience. But overall, it should be more or less similar.Step 1 - Apply for Learning License.Apply online, if you can. Or visit your local RTO office and fill up form and submit. Make sure you have all the documents; Aadhar card, PAN card, 4–6 photos, Address proof (Passport, Electric bill etc). If you are applying online, you have to upload scan copies some of these documents. There is a form which talks about medical conditions. It is just a self certified medical certificate. Doctor’s sign or stamp etc is not required. Don’t waste your time and money to get Doctor’s sign. If you are applying online, you have to make an online payment. Make sure to download and print auto-generated application form and payment receipt.You have to select a slot to appear for computerized test at RTO. This is mandatory and you have to go to RTO. Study the rules before you appear for the test. It is relatively simple exam but you still have to pass. If you fail, you have to book another slot on some other day and appear again. There is lots of online help to study the rules. You have to get 80% to clear the test. I don’t exactly remember but I think there are 10 questions and you have to get 8 right.Step 2 - On the day of online test for LLDocuments to carrya. Printed application form (sign wherever mentioned)b. Receipt of the paymentc. Medical form (sign wherever mentioned)d. ORIGINAL Aadhar card & Aadhar card copye. ORIGINAL PAN card & PAN card copyf. ORIGINAL of address proof you uploadedg. 4–6 photosh. Any other form generated during online booking (This is a bit in-consistent)i. Stationary (Stapler, gum, pen etc) - Believe me they will be life saverYou may have selected a slot to appear for test, but it is just indicative. Nothing will happen according to time. You still have to stand in a queue anyways. I hope situation will improve in future.The first thing will happen is the verification. There will be a guy who will verify your documents and make some entry in the computer which means that you have appeared for the exam and you will get a token number. Until this guy arrives, nobody will know what to do. You will be asked to sit in a waiting room and you will keep wondering what’s next. As soon as this guy arrives and announces something, just run towards him and stand in a queue. No matter what your slot is, your verification will happen based on who was first in the queue. There will be some agents who will try to sneak their candidates directly to this guy. Nobody will say anything and you will realize there is a second queue for the candidates who come through agents.Once your verification is completed, now wait for your token number to appear on a display. As soon as your number appears, go towards another person (most probably a constable) who will again verify your documents against the ORIGINALS. Please note that color xerox or downloaded documents are not accepted.After documents are verified, you will be allowed to enter the exam hall. Examiner will guide you and start your online test. If you clear the test, you will know immediately after you answer the last question. If you pass, immediately the examiner you will get your learning license. The examiner gives some commands after you pass and the prints are generated. They laminate the prints and hurray…. You cleared second hurdle. If you have applied for multiple categories, you will get one license for each category.PLEASE MAKE SURE YOU GET YOUR DOCUMENTS BUNCH BACK AND RETAIN IT, BECAUSE IT WILL BE NEEDED AT THE TIME OF PRACTICAL DRIVING TEST.Step 3 - Apply for Driving LicenseYou can apply only after 1 month of getting your LL. Make sure you are confident enough to clear the driving test. If you are applying for more than one type of vehicles, you can apply for different slot or different day. Remember that you have to apply for the practical test physically at the same RTO from where you got your LL. Also, you have to carry your own vehicle and make sure all original documents of the vehicle (RC, Insurance, PUC) are with you. Again, make the payment online and make sure to download and print all the documents that are generated (Application form, payment receipt or any other form).Step 4 - On the day of Driving TestDocuments to carrya. Original documents bunch that you got back after appearing for LL test (I have seen so many people did not carry these and they were not allowed to appear for the test)b. Learner’s Licensesc. RTO does not provide any vehicle. You may find people giving the vehicles on rent for applying for the test. But they charge too much. Not worth. I applied for LMV and MCWG in the same slot, but carried only my car. I made a couple of friends while waiting for my test and they allowed me to use their scooter for MCWG test. I was lucky here, lol.Also, you have to produce vehicle documents (RC, Insurance, PUC) before starting the test and also enter the relevant information on the form. If you are lucky, you may get away with showing the original documents.In Wadala Mumbai RTO, the test happens on an open road which is around 500 meters away from the RTO office. There are no directions mentioned anywhere. You just have to ask someone and find it yourself. This is really strange. They earn so much and still don’t have a facility to conduct test. Everything happens on open public road.Here again, it does not matter what slot did you book, you have to wait for the Examiner to appear and then it’s all chaos. The place will be full of agents and middle-men. They know exactly what to do and that’s why their candidates don’t have to worry. But if you are going without any agents, you have to continuously check for the arrival of examiner and stand in a queue as soon as he arrives. Just before the examiner arrives, there will be few RTO people who will verify your documents and ask you to sign on a register. Once your documents are verified, he will write something on application form and ask you to stand in a queue in front of the examiner.As soon as your turn arrives, examiner will again check your documents and your test starts. After this everything is very subjective and there is no straight rules or strategies that will or will not work. There are lots of factors that will decide what exactly you have to go through. I heard in some of the RTOs, examiner sits beside you in your vehicle and give you instructions. Depending on your performance, you will be judged. In my case, nobody sat beside me. Examiner just asked me to drive my car around the building and come back at the original place. I just drove around the building without anybody noticing me and that’s it. Since I applied for MCWG also, next thing he asked me was to drive my scooter in a straight line and take a U turn and come back. I asked him whether I passed and he said yes. Since I could not believe it, I asked him again if there is anything else I have to do, he said no. That was it for the day.I checked everyday on the RTO portal and I could see the application moving every 5–6 days. Finally one day I saw my driving license dispatched through India Post and I was relived. It took 15 days for my DL to arrive after my test.
