By trained intermediary, she means a licensed psychologist (as she herself is) who is trained to recognize and deal with the issues that people with Borderline Personality Disorder or Narcissistic Personality Disorder create in marital or family situations.Why did the therapists you approached turn you down?Dr. Fjeldstad is certified as a marriage and family therapist and obviously also did some sort of training in Borderline and Narcissistic Personality Disorders. Finding someone who is trained in all of these things is very rare. The therapists you have approached were correct to turn you down. They knew they did not have the necessary educational background and experience to ethically even try to do as you asked.What type of training does the therapist need?They need to be a licensed psychologist or psychiatrist.They need 3 years post-graduate training in some form of psychotherapy that can be expanded to work with BPD and NPD. Dr. Fjeldstad is a licensed marriage and family therapist. This is what the initials LMFT after her PhD mean. She appears to be utilizing her base training in marriage and family therapy as her therapy platform to build on.They then need 3 + years of training from a reputable post-graduate training institute in the diagnosis and treatment of Borderline and Narcissistic Personality Disorders. I do not know whether Dr. Feldstad did a formal training in personality disorders or is self taught.They need 3-years post-doctorate training in couple and family therapy from a reputable training institute.They need to study the art and science of mediation.They need 5–10 years experience doing mediation in families where someone has BPD or NPD.The above takes great dedication, enormous amounts of work, and a huge investment in time and money to accomplish. And, training alone is not enough. It also requires skill, creativity, and high intelligence.Asking your average psychotherapist to do all of the above is a bit like expecting your GP physician to do brain surgery. You are highly unlikely to find many people with the necessary training. The term “therapist” can refer to anyone who works in the mental health field in clinical care. For example, just in my state, there are Psychologists, Doctors of Psychology, Licensed Professional Counselors (LPC - focuses on mental health diagnosis and general mental health), Licensed Marriage and Family Therapists (LMFT- usually family system oriented, focus on couples, family), Licensed Clinical Addiction Specialists (LCAS - Addictions focus, substances, behavioral, treatment and general mental health ), and all may refer to themselves as therapists. Psychologist all have specializations as well. For example, forensic psychologists may assess a client for an attorney and testify in court.Of course, sometimes when I share I’m a therapist, people sometimes assume I’m an Occupational, Physical or Massage Therapist. So, I may refer to myself as a Mental Health Counselor, Mental Health Therapist or Psychotherapist. It’s all the same thing.My specialization is as a CSAT - Certified Sex Addiction Therapist. This means I have lots of extra training, education and experience on how to treat sex/porn and other behavioral addictions, the families and partners of people with those addictions. For example, if you want help with your marriage, you wouldn’t see an addiction’s specialist. However, marriage and family therapists refer clients to me if there is an addiction suspected or revealed.Psychiatrists are medical doctors and they can prescribe medication. General therapists can not prescribe medication without additional degrees and licensing. So, if your LMFT gives you adderall for ADHD, run away.
