What is a doctor’s weirdest case of a patient accurately self-diagnosing?
Two answers for this.First, I was on call in the emergency room as a resident and a family brought in a gentleman and said they wanted a CT scan of his brain because they thought he had a brain tumor. I asked why they thought that, and they said because he was nice. I stood there confused until they said that he’d always been a really nasty person and now suddenly he was very pleasant, so they were sure he had a brain tumor. I stood there dumbfounded. My examination was normal, but I’d always been taught to listen to patients and their families, so I ordered the CT. My attending saw the order and started yelling at me to not waste time and resources on such a ridiculous thing, but it was too late since he was already being wheeled into CT.CT result? He had a large frontal lobe brain tumor! (Frontal lobe tumors may cause personality changes.) So yes, the family, in this case, diagnosed the patient!Second case. I was on duty in the ICU and a patient told me she thought she was going to be dead in the next 24–48 hours, so she wanted me to call all her family and ask them to come see her to say goodbye. Mind you this particular patient wasn’t doing too poorly and I had no anticipation that she was on death’s door. Also, not all her family was local, so calling them in meant some of them flying cross country! But again, I tend to listen to patients, and I figured that the worst that could happen is her family would come see her and she would live! Plus part of me was thinking that maybe she had to use the threat of imminent death as a way to get her family to visit her, which was sad.So I started calling the family and over the next 24 hours they all started arriving. Well my attending found out and once again I got reamed out. He accused me of scaring everyone and telling the patient that she was going to die, when clearly this was not the case in his opinion. I tried to point out that she was the one who decided she was going to die, not me, and that I was just following her wishes. I told the family members that and that she was the one who requested their presence, not me.Result? Within 24 hours of everyone arriving to say goodbye she died.The moral of these stories? Always listen to patients and their families!
