The thing is when doctors are travelling, they can't carry heavy medical equipment. Many doctors may not be well versed with emergency medical care in their regular practice. They may not even specialize in the specialty where the particular medical problem may arise. What a doctor may possess is medical general knowledge from medical school that may be invaluable in a plane full of lay people.Many years back during my Ophthalmology residency in Kolkata I was travelling from Kolkata to Bangalore, a gentleman a few rows behind me had an acute asthmatic attack. The crew member asked on the address system whether any doctor was on board. I identified myself as a doctor and I came to know from the accompanying family that he was a known asthmatic who was not responding to regular salbutamol nebulization (a medication modality). I asked the crew member for the emergency medical kit which is supposed to be in every plane. I took charge of the situation, gave him an injection of adrenaline and hydrocortisone after which he improved dramatically and we could continue on the journey without a problem. There are lot of useful emergency gear including the Automatic external defibrillator (AED) and several life saving medications in a plane. This intervention was successful because I used common sense, some basic medical knowledge and luckily for the patient, the appropriate medicines were available.The code of ethics dictate that a doctor is obligated to treat a patient in situations of emergency when he/she has all the tools to save the patient's life. When no basic medical facilities are available a doctor can't do much just by being there. A heart attack or myocardial infarction is one of such situations.The event I described was many years ago and I then had an ideal view of the world where by rising to a need, my actions would be seen in good faith. Given the current state of affairs with frequent assault of doctors in emergency rooms when the outcomes aren’t favorable- no matter what the circumstances- today I would probably be hesitant to treat anyone in a plane with such limited resources.
