Why do doctors have bad handwriting skills?

‘Your language is good but running is bad; try to improve your running.’​I was a frail kid and was highly susceptible to any infection that walked past the school corridor, making my dad visit the local doctor, and my mom the local temple, with competing frequency.​I was never good at sports, so my teacher was obviously not referring to my physical education but pointing to my poor cursive handwriting in the school note-book. Those days ‘cursive handwriting’ was loosely referred to as ‘running hand’ or abbreviated to ‘running’.​But despite her encouragement, things didn’t go exactly as my teacher imagined. Admission to medical college brought about a total change of lifestyle and shattered my time-management. A hectic schedule of back to back classes, interspaced with bedside clinics, exams popping its ugly head at every turn, led to progressive increase in stress, a matching increase in volume of notes and a progressive decline of handwriting.​The problem multiplied several folds, as I finished college and joined for post graduation course of MD in internal medicine in Trivandrum Medical College. The medical journals carried review articles in huge numbers, each bringing out conclusions diametrically opposite to its predecessor. As a medicine postgraduate, our job was to keep note of all those and present them in the weekly ‘journal club’. Every day ended with a huge pile of notes, mostly unreadable, stacked up on the study table. My running hand by now had deteriorated to a scribble. But still I read journals and made notes like crazy.Most notes could not be located when needed, those few which were traced could not be deciphered even by myself, making it more challenging than a Dan Brown novel. But despite these failures, a faint trace of that article, its context, remained somewhere deep inside the brain.It is proven that writing a note, scribble or whatever, leaves an indelible mark somewhere deep inside the amygdala and hippocampus, areas of brain which consolidate memory, springing back some recollection.​And then for people like me, who were congenitally bad at running the hand, came a breakthrough. It was called ‘Photocopy’.​The availability of the technology excited us, the initial high cost made us pick and choose the ration, but as every street corner shop got a ‘Xerox’, costs dipped, every journal article was photocopied.​It was thought to be a game changer, but nothing of that sort really happened.​All photocopies were quarantined on the study-table for couple of weeks, then subsequently shifted to ‘under-the-cot’, for a prolonged hibernation and later sent to a submarine in Siberia, never to resurface again.​Our legendary professor of medicine Prof. K.V.Krishnadas was strongly against photocopy and considered it an expensive way for medical information to bypass the MD students brain without leaving a trace; making no impact whatsoever at all. So we needed to smuggle the copies to our friends out of sight of the professor.​By the time we were addicted to photocopy and degraded our running hand to crawling on four, a bespectacled gentleman named Bill Gates offered us a life changing gift that changed the transaction of data for ever. The dirty scribbling gasped and faded away, there was no one to write even a decent obituary. Now there was no need for running hands, crawling or scribbles, just hit the right ‘key’ to write, the auto-correct made sure that the grammar teacher lost her job for good.​A recent study has shown that students who took notes in a laptop or tablet, were faster than those who wrote in cursive. Also everyone could read it correctly, and if the content was sufficiently succulent, it would become viral at the hit of another button. But the flip side is, when the students were asked to answer a set of questions on the said topic, those writing in cursive fared far better, because it did enter their brain. The simpler, faster keyboard hit, did not impact the brain that much.In an era where ‘how to write a love letter’ returns a million hit in 4 seconds, a cursive handwritten letter is like a hand crafted gift, offered at a simple dinner at home. The shiny, backlit keyboard is akin to the 5-star hotel, on a dance floor with huge razzmatazz; it steals the show but not the heart.​Finally, the keyboard ‘hit’ won, and the ‘running’ hand dropped-out.-End Note -​It is said that there are two reasons for a bad handwriting; either poor dexterity of the hand, or a ‘high-speed’ thinking process thats too fast for the hand to match.My hospital pharmacist swears that it is the former, but deep in my mind I still believe that it is the latter.(From my own article in the HINDU)

What is the saddest case you have had as a doctor?

While on an assignment at Temple University Hospital, which is a teaching hospital, I noted “The Case of the Week” on the radiology department billboard.It was a plain x-ray of an infant, with what was plainly a gassy abdomen. I halted and reviewed the film and my eyes teared up. Bad enough that much of the intestines were gassy, it was also apparent that a section had decompressed and the abdomen itself had gas in it, suggesting intestinal rupture.The chief radiologist happened to walk by and noted my viewing the film and reaction and asked me what I thought.I explained the above and asked, two words, “Two hours?”, to which he sadly confirmed.A four day old child had suffered from undiagnosed imperforate anus, with no fistula formation, leaving the intestine completely blocked. By the time he was presented for diagnostic imaging, his intestine had ruptured. He survived for two hours after the image was taken.Doctor had me present the case, as none of his residents had managed to accurately diagnose what was wrong with the child. That caused a number of residents to redouble their studies, as I was only an SF medic.
