What is the importance of a nursing care plan?

SO EVERYONE IS ON THE SAME PAGE WHEN IT COMES TO A GOAL.A care plan flows from each patient’s unique list of diagnoses and should be organized by the individual’s specific needs.Continuity of care. The care plan is a means of communicating and organizing the actions of a constantly changing nursing staff. As the patient’s needs are attended to, the updated plan is passed on to the nursing staff at shift change and during nursing rounds.Care plans serve as a guide for reimbursement. Medicare and Medicaid originally set the plan in action, and other third-party insurers followed suit. The medical record is used by the insurance companies to determine what they will pay in relation to the hospital care received by the client. If nursing care is not documented precisely in the care plan, there is no proof the care was provided. Insurers will not pay for what is not documented. The purpose of students creating care plans is to assist them in pulling information from many different scientific disciplines as they learn to think critically and use the nursing process to problem solve. As a nursing student writes more plans, the skills for thinking and processing information like a professional nurse become more effectively ingrained in their practice.

What is the weirdest thing you have ever experienced?

Thanks for the A2AThis gives me the opportunity to write about an experience I had that's stayed with me.It's one of those things that keep popping back into my mind occasionally. One of those intense memories we can form.I'm a nurse. I've worked in a whole bunch of areas. I've worked other jobs too, I've been a waitress, a day care boss, a travel guide, a theatre prop tech.... I've been around the block.15 years ago I went back to nursing school in Canada to get my career in that field going again. After a ten year hiatus. I'm a very practical person. I don't believe in miracles. Or so I think.Whilst in school I was allowed to work as a student nurse only in certain areas.I got a position with a home care agency and I was assigned to care for a gravely ill and dying woman. This is not something I'd done before. I mean, I knew all the technical in's and out's.I had it all down on paper. I had the perfect and pristine outfit and ambition to spare.I had care plans and phone numbers and charts and drugs to administer. It all seemed set and easy. She was accommodated in her own home, and there was family around, which isn't something everybody has.So I started to go there every day. In the beginning she was still lucid at times, but always in pain, always fighting. It was a nightmare to soothe her, to wash and turn her. To ease that suffering.I felt so helpless. And over the weeks the family began to trust me and leave her and I alone sometimes. This went on for several weeks and of course she deteriorated but did not die.In that situation the fear of timing is huge.Nobody wants to go. But nobody can predict or make it stop either.Anyway. So one afternoon I came to start my shift and noticed an imperceptible change in her. A gut sense, a feeling . And I asked the parents to stay the night. By that time she was in so much pain and unconscious. The family had shared with me that she had always been a fighter, that she wasn't one to give up.And I knew what that's like. I'm the same. Then, around two in the morning, she simply became terribly agitated and struggled. I knew she was in pain. I knew I had exhausted all the meds I was allowed to give her. I had positioned her as carefully as I could, I'd washed her and I'd done anything I could think of. There was nothing else. And still she struggled.I don't know why, but at that moment I leaned down close to her face. I kissed her cheek. I was so close to her ear. She smelled of baby powder.....And then I whispered ...." ....... It's ok. It's all ok. I'm here. Mom and dad are sleeping next door. I know you hurt so much, and I know you are afraid. But it's ok.Let go. Just let go. Let it go. We are all here. You are not alone. Let all of it go. It's ok to die. You've done enough."I whispered that gently into her right ear. I remember it like yesterday. Then I slowly stood back and looked at her.And then it happened.Just like that. She calmed. She simply calmed. Her breathing slowed to completely normal for about a minute. Her heart rate settled. And then that passed and it went into what is known as Cheyne Stokes breathing.Which signals imminent death.I woke her parents then, I woke them gently and said it was time to sit with her. They got up very quietly and asked if I was sure. And I said I was and to go and sit. It wouldn't be long.So they did. And it wasn't.Five minutes later, after a months long final struggle, she stopped breathing whilst each parent held a hand.Ok. So now comes the strange part....Thanks for reading this far people....The second she died....And it was all three of us who felt this..the room suffused with an odd energy. With this almost imperceptible breath. This gently caressing hush. Time and all worry ceased. We felt as though we were being touched by feathers lighter than air.By dandelion fluff blowing onto our skin.Seconds went by during which nobody moved. We just shared the same space but the space became lighter. It was so incredibly creepy. So incredibly beautiful. It was as though she was everywhere, she was flooding every essence with her soul.I'm convinced that in fact she did.I need no explanation.Nor do those parents. Not ever will I. That whisper and caring set her free. And I think she thanked us for it.....I have seen other people die. It happens everyday. But I have Never felt that energy in that way again.Maybe smittens of it here and there. But never that intense and strange. And to remember it is a wonderful thing.Gives me goosebumps.

Why Do Our Customer Attach Us

I haven’t a clue why all these other reviewers are so unhappy. CocoDoc’s Filmora editing program is fantastic and exactly what I needed when I bought it for a one-time, lifetime cost of $60 late in 2017. When my laptop died a few weeks ago, I needed to reinstall ALL of my apps. The one for Filmora kept asking me to pay for it again, which I of course did not want to do. I finally reached tech support at CocoDoc and they sent me easy to use and clearly stated instructions and codes to reinstall it. That was yesterday and I couldn’t be happier! If you want to see examples of editing done using Filmora, my YouTube channel is Kristene’s Dogcationers. (Those videos are shot specifically for the dog owners - and to give me practice editing.)
