I'm going to assume that my floor is awesomely staffed so that I can pretty much hit everyone at once.While I'm going over my patient's OR consents and orders I'll ask my resource nurse to go and evaluate my patient in pain. I'll deploy my CNA to assist my bathroom needing patient. I'll poke my head in to my feeling sick patient and eyeball him, making sure he's not vomiting, evolving an MI etc. I'll make sure his call light is within reach, make a mental note of the time, and let him know I'll be back but to call if he's feeling sick in the meantime.I then grab my supplies and get my OR patient up to the bathroom while I finish the checklist and meds in the room.The OR circulator shows up, pleased as punch that I've made their job so easy, they high five me.I document my patient up to the OR, do a quick check with my resource and CNA and then go sit with my sick patient. Or, I might go sit with my sick patient while I document. That's probably more likely.Of course, that's not what happens in real life. In real life I would be understaffed and taking this on alone.In that case I'll go and get my bathroom patient up first, make she he's stable and go next door to my pain patient and ask her to rate her pain.Next, I'll get my bathroom patient back to bed and go and check on my not feeling well patient. He doesn't look great. Crap. I'll slap on his pulse ox and make sure he's satting ok. While this happens I'll log on and look at my OR patients orders. Double crap!! He needs like three IV push meds and a prep I didn't know about.My patient is satting in the low 90's, I put on his nasal cannula at 2 liters noticing my OR patient has 1 minute to be ready, I can hear the elevator doors open and a wheelchair rattle out.Mr. Notfeelingwell looks a tad better and he's at 97%. I hand him his call light and let him know I'm stepping out, but to call me...if he feels like he's about to die, I mean, feels worse.I meet the wrath of the OR nurse.I'm feeling a tad super human today so I kindly enlist their help to finish getting the patient ready. He gets them up to the bathroom while I book down the hall to get their pre-op meds. A call-light goes off as I pass Mrs. Myleghurts room."Where's my pain medicine?"I pull my OR meds, grab the prep and consents and book back to the room. I poke my head into Mr.Notfeelingwell's room, he responds, but his pulse ox is in the 80's.I literally throw everything at the circulator, tell him I have another patient who is circling the drain, gotta go.He asks for my full name to write me up. Fabulous.Back into my sick guy's room, he doesn't look great. My gut is telling me everything is wrong. I get his blood pressure and call out for help.The charge nurse shows up, but to ask me why I haven't medicated Mrs. Myleghurts, she's called again. I SBAR the crap out of this and ask for a RR team.After my patient codes and is transferred to the unit I'm left with a pissed off OR, a cranky pain patient, and a charge nurse (who had been MIA) who's asking why I didn't call for help sooner.I start banging my head against the wall...
