It doesn’t look good. One way to judge the potential of a drug target is to look at the trials against it. Ask yourself:Did it work? Was there any clinical benefit in using the drug?Did it do what it was supposed to do? If it failed, why did it fail? Was it because the pharmokinetics were unfavorable or the drug did not effectively engage the target in vivo? Or did the drug do exactly what is was supposed to and there was still not a measurable effect?The results from the Merck EPOCH trial of Verubecestat (MK-8931) suggest BACE inhibitors may fall in the latter category. MK-8931 was very effective in inhibiting BACE in vitro:[1]and dropping A[math]{\beta}_{1-40}/A{\beta}_{1-42} [/math]levels in rats:[2]and humans but it still did not have any clinical impact on the disease.[3][4] It doesn’t seem BACE inhibitors, or techniques aimed at removing monomeric A[math]{\beta}_{1-40}[/math]/A[math]{\beta}_{1-42} [/math]in general, will be successful (witness the failure of the Solanezumab trial).[5] Interventions based on restoring the normal function of A[math]{\beta}_{1-40}[/math]/A[math]{\beta}_{1-42}[/math] [6]also aren’t looking good (the patients didn’t get worse, which would be expected in this theory if soluble A[math]{\beta}_{1-40}[/math]/A[math]{\beta}_{1-42} [/math]fell dramatically ).It’s unclear what MK-8931’s failure means for other amyloid based Alzheimer’s interventions. Aducanumab, an anti-amyloid antibody which targets both soluble and aggregated forms of A[math]{\beta}_{1-40}[/math]/A[math]{\beta}_{1-42}[/math], slowed memory loss in a small clinical trial,[7][8][9] one of the very few successes of any type in the field.Why has aducanumab apparently succeeded while MK-8931 failed?I don’t know. I don’t think anyone else does either.Some possibilities:The apparent success of aducanumab is an illusion caused by the small numbers of patients in the trial. The figure above is pretty convincing through.The aducanumab trial identified a subpopulation that the MK-8931 trial missed. In particular, The aducanumab involved an amyloid positive PET scan while the MK-8931 trial did not. The clinical criteria in the aducanumab trial may have selected for less advanced cases of Alzheimer’s that are easier to treat. If, as the amyloid hypothesis suggests,[10] amyloid aggregation is driving a second process that is actually responsible for the majority of the damage in Alzheimer’s, it is unlikely that BACE inhibitors will have an impact on advanced cases, especially if the second process is self-sustaining like tau hyperphosphorylation and aggegation.[11]MK-8931 targets A[math]{\beta}_{1-40}[/math]/A[math]{\beta}_{1-42}[/math] production while aducanumab directly targets the aggregated forms of A[math]{\beta}_{1-40}[/math]/A[math]{\beta}_{1-42}[/math] responsible for neurotoxicity. BACE inhibitors as a clinical therapy rely on the existence of a dynamic equilibrium between soluble A[math]{\beta}_{1-40}[/math]/A[math]{\beta}_{1-42} [/math]and whatever aggregated form is causing the damage.[12] If the off-rate or clearance rate of soluble [math]{\beta}_{1-40}[/math]/A[math]{\beta}_{1-42}[/math]is very slow,[13][14][15] [16] it is unlikely BACE inhibitors will have an impact.
