First evidence that e-cig use increases heart attacks, independent of the effect of smoking cigarettesEvery time I have posted a comment on a new study showing that e-cigarettes adversely affect blood vessels and blood in ways that increase risk of a heart attack, a friend and colleague who remains part of the (shrinking) collection of e-cigarette enthusiasts emails me and with the comment that, “if they are so bad where’s the evidence that e-cigarettes increase the risk of a heart attack?”The first evidence just appeared.Using the National Health Interview Survey (NIHS), a large national survey done in the US, Nardos Temesgen and colleagues at George Washington University, found that the odds of a heart attack increased by 42% among people who used e-cigarettes.This increase in risk was on top of the increases in risk due to any smoking that the e-cigarette users were doing. This is a particularly important finding because most e-cigarette users are dual users who keep on smoking at the same time that they use e-cigarettes. What this means is that dual use of e-cigarettes with cigarettes is substantially more dangerous than smoking alone.The assumption of most people – including me – has been that e-cigarette use would not add much risk to cigarettes. That is not the case. E-cigarette use increases the risk of a heart attack about as much as having diabetes.The work also controlled for a wide range of confounding variables. In addition, the risks associated with other factors (like smoking, diabetes, and high blood pressure) in this study were similar to those established in earlier work. Nonetheless, a recent study showed both E-cigarettes and traditional cigarettes cause an increase in oxidative stress and endothelial dysfunction, however this effect is less pronounced with E-cigarettes. Currently, there is a limited study that shows the impact of E-cigarette in the cardiovascular system. Therefore, data from the 2014 National Health Interview Survey (NHIS) was used to evaluate the effect of E-cigarettes on the cardiovascular system, specifically the effect on myocardial infarction (MI).Methods: Analysis of the 2014 National Health Interview Survey (NHIS) database was performed to examine the effect of E-cigarettes on MI. Initially, subjects were assigned to one of two groups: those with a history of MI and those without a history of MI. The t-test and chi-square test were subsequently applied to compare the different demographics and health characteristics between these two groups. A logistic regression model was then used to measure the association between E-cigarettes and history of MI. Data was adjusted for multiple risk factors for MI including age, gender, race, body mass index, income, the status of smoking cigarettes, and history of hypertension, diabetes, and hypercholesterolemia.Results: A total of 35,156 subjects were included in the final logistic model. How did you become an entrepreneur?

Last December I left a job managing the largest sales territory ($37M) for Altria Group. Altria Group is one of the most iconic CPG companies of all time, and held the trophy for highest ROI stock of all-time.I was particularly proud to have had 16 years of education culminate in a sales job of that caliber. You see, I was homeschooled for my first 7 years of school. I felt like I was playing catch-up socially for a decade.Over my two and a half years with the company, I grew bitter about the job. In case you’re not familiar with Altria, they are the parent company of Philip Morris USA, US Smokeless Tobacco, John Middleton Cigars, and some e-vapor companies.I always felt tobacco should be illegal. However, I was able to look past my personal stances for the compensation. I reasoned that I was outvoted— that this was the result of democracy & capitalism.Over my tenure with the company, my perspective changed.While management raved about a future of healthier alternatives, it became clear to me this was not their intention. Altria lobbied the FDA to set Pre Market Tobacco Application standards that made it almost impossible for e-vapour companies to start-up, and even survive. A $500,000 review process would be required for any new variation of flavour/size, any new device, and any new part. Now they were the only ones who could afford nationwide distribution, and pushed an intentionally unsatisfying product.Juul developed an incredibly satisfying alternative product. It uses salt base nicotine rather than freebase, so the buzz hits you like a real cigarette. Electronic cigarettes are far and away the most effective smoking cessation tecnique. I suspect this is especially true for Juul. Altria never seemed interested in acquiring Juul and helping America move away from combustion. Instead they sued Juul for hundreds of millions of dollars over a weak trademark infringement case. Juul is now valued at $15.9B.Altria touts their efforts to bring the combustion-free, highly acclaimed iQos device to the United States. I suspect they are intentionally dragging their feet in execution, so they can tell investors and the FDA they can be trusted.They fight dirty when it comes to fighting excise tax increases. I wouldn’t see this so negatively if I didn’t already feel strong convictions listed in the bullet points above. It was clear the company’s intention was not to allow any healthier alternatives thrive in the marketplace until they were forced to, and then they would continue to monopolize.I felt like a sell-out for choosing compensation over values. Still, as I considered other employment prospects, I couldn’t think of many corporations I felt were doing more good than harm.As these issues wore on me, I knew I probably wouldn’t be able to stay much longer. But I’ve made tremendous strides toward ‘finding my purpose’ just by seeking out challenging work.There is something so satisfying about delivering results in niches that are so complicated. We are doing work that we can be proud of, and that’s what’s saved me from this rumination over ‘finding my purpose’.I escaped the 9 to 5 at just 24 years old and guess what?
