How do medical doctors note miscarriage or abortion in a medical report?

Spontaneous abortion or miscarriage = “SAB”Elective or therapeutic abortion = “TAB”OB/gyns incorporate these into the Gs&Ps: Gravidity and parity - Wikipedia

Has a therapist ever told you something completely unexpected?

It's “most people in the medical field are awful" storytime once again! Many years back I'd had a miscarriage that devastated me. I tried everything to crawl out of my depression but nothing was working so I sought therapy. I began going to a lovely older woman, and had begun to make a bit of progress when the office abruptly dropped my insurance. There was no way I could afford $180 a week out of pocket. After calling a dozen places I finally found a doctor who had an opening that wasn't 6 months out. It was a man, which gave me pause, but at the same time I felt like I would backslide if I just stopped going entirely. My therapist said she would send some notes to him.The day I arrived for my appointment, They had me fill out the usual paperwork. Asking about my job, what medication I take, general health history. It asked if I had it history of addiction (I don't) It asked if I was on psych medications and I was not.So I immediately meet this gentleman and he plops down with his back to me to access his computer. Okay fine, perhaps he was looking at my file on his computer or something.First words out of his mouth. “You are clearly overweight, so I can skip weighing you."Note that he was a therapist. At no point would he need to know my weight to prescribe medications.I was a bit taken aback, I didn't know what to say. Next he spun around to make eye contact and said, “Your paperwork says you are a writer. Is that true?"I said yes, I've been published, but I am by no means JK Rowling. At the time I was writing YA fiction. He leaned forward and said, “I'm going to search your name in my computer right now, if you are really published you will come up, So if you're lying you better tell me now.”At this point I was confused and getting upset. It's not like I had said I was the Queen of England, and I was overwhelmingly uncomfortable. He Googled me and there I was, picture and everything on my publishers website, my website, interviews. He looked disappointed if I'm being honest. And this is where our story comes to an abrupt halt, as the next words exited his mouth.I can tell it's you from the picture, despite all of the Photoshop you used to make yourself look smaller.I was speechless and I picked up my things and left. At no point did he try to stop me.The entire experience was completely surreal. I'm not sure why he felt the need to validate my identity. I'm not sure why he read my doctor's notes and decided to focus on my weight. So many years later I look back and I still can't figure out what happened.I ended up paying out of pocket to see my old therapist about once a month, basically only accessing her when I felt that I was headed to a dark place that I could not control. That whole nightmare experience was certainly unexpected.***Edited to add:I HAAATE how many people can relate to this story. No one should have to go through this kind of discomfort when they are already suffering. It is so unacceptable the way people are treated in this country in regards to their mental health. Especially women, especially grieving women, especially overweight women in both medical and mental health. To answer a few questions that have been asked repeatedly below. The Dr. was an Indian man in his 50s. I only mention his ethnicity because both my old therapist and a few other people who I have told this story to have used cultural differences to explain where his focuses were. I don't care if he was a purple people eater from Mars, I don't think that is an acceptable excuse.I went back to my old doctor and told her what happened. She was not surprised, as she had heard negative things about this doctor but did not want to “poison the well" if on the off chance I liked him. Frankly I felt very betrayed by her withholding information and I told her that. It took a while before I could fully trust her again. She remained my doctor for 8 years until she ultimately retired for health reasons. Unfortunately the past couple of years I haven't had a therapist because I am absolutely terrified to try and find a new person. I am however doing much better now.

What types of evidence win court trials?

Let me tell you about a time I lost faith in evidence, justice, myself and juries.I defended a client indicted on a Rape charge. It was one of those few cases when I felt not only was my client Not Guilty I was actually convinced he was innocent. Yes there is a big difference between the two terms.I thought the trial went well:I caught the alleged victim in clear multiple lies on the stand.I produced a believable witness on the stand who testified that the alleged victim admitted to her that she made the entire thing up out of spite and for attention. My biggest piece of evidence however were authenticated in-depth medical records which included: DNA report, a rape kit, doctors notes, nurses notes, and many contradictory statements by the victim INCLUDING RECANTATIONS all made close in time (hours) after the alleged incident.my defense screamed reasonable doubt loud and clear.in my closing I went over all the evidence that she was lying in detail. I implored the jury to go over all the evidence especially the medical documents in detail. I asked them to please read the documents out loud. Surely at the least this was enough for reasonably doubt. So I thought.while the jury was deliberating I spoke to the prosecutor and asked what kind of sentence she would be asking for if the verdict came back guilty. She laughed. “There’s no reason for that as there no way any jury could come back guilty on this one” her exact words.the only weakness in the case was the physical look of my client. A nice enough guy but if you were casting a movie and wanted someone to look the part of a rapist I suppose he had that look. But surely and legally this was not enough for guilt beyond a reasonable doubt? So the jury found him guilty. It’s the biggest loss that haunts me everyday. I often sincerely think if he had had a better lawyer he would be free today. He is not.the ultimate insult. Is that when the court officer brought out the evidence after trial I noticed the folder containing the sealed medical documents HADN’T been opened. The seal had never been broken…the jury hadn’t even bothered to read the documents… Damn it, I mean find the man guilty but at least have the decency to look at the evidence first for 15 minutes! What’s wrong? Worried about seeing something that would change your mind?writing this now all I can think is my God what a miscarriage of justice. How could they not even bother to look at the evidence with so much as stake? Didn’t they remember their oath? Think about what I’m writing. A Man’s life and freedom is on the line. Or if you want to look at it from the other way, a woman may have been the victim of one of humanity’s most horrible crimes. Think about it now. And your job as a juror is to decide, based on evidence who is telling the truth. As a juror you took one of our most important oaths…and 12 jurors sat in a closed room and didn’t bother to take the time to open an envelope and look at about 20 pages of vital evidence…to make up their minds one way or another…I put everything on the record. I hope everyday to hear the cases was overturned on appeal.I had a mentor. In my opinion he was the best trial Attorney in Massachusetts. He once told me “The best we can do. The most we can do as lawyers, is put our faith in the jury. And hope they get it right. Most of the time they do.” I suppose this is true. What other option is there?sorry for all grammatical, spelling and syntax errors. Haven’t the heart to correct them. Much like the jury.thanks for letting me whine

It has a friendly interface, it is specific in its functions, it seems great to be able to highlight or underline texts in the document just by pressing a button in the options menu. I love the transition from one page to another in a document when it is in 3D View. Convert Word documents to PDF without any difficulty
